devastated :( low mature %

Really devestated at ER results due to low mature % (11 retrieved, 3 mature) and looking for opinions on how the cycle was managed.

This was our first IVF cycle and we chose a clinic in Prague after doing some research. My protocol was originally 300ml Gonal for the first 3 days (starting day 2 of my cycle) then 250ml for the next 7. Certotide 25mg for days 6,7,8,9 & 10. Our day 7 scan (day 6 of stims) showed 14 follicles with the biggest 9 measuring;

17.3, 16.8, 16.0 (x2), 15.6, 15.3, 15.2 (x2), 14.0

After we gave them these results we personally thought they seemed like they were growing fast considering the early stage and they brought my second scan forward from day 9 to day 8.

When we arrived in Prague we had this done and they said things were going ok but didn't really give us a bunch of measurements, just said they expected to retrieve 10 or 11. They did reduce my Gonal from 250 to 200 for the last 3 days. We were surprised when they said that they would stick to the original ER date as we expected it to be sooner but trusted their judgment and expertise.

We just had our retrieval today (7 days after the UK scan with the measurements above) and of 11 retrieved only 3 were mature. This was quite a shock and definitely not what we expected. We didn't really get solid answers in the meeting after as to why they thought this was and the language barrier made it harder, plus the state of shock and finding it hard to focus. We can't help but wonder if they waited too long to do the retrieval.

Just wondered if people who are knowledgable would give their opinions as to if they think anything was mismanaged given the above scenario.

Hello, sorry this has happened, it's always really upsetting when you get a shock like this. I've had experience of lower maturity, and although I'm no expert, my understanding is that the delay won't have made this worse - if anything it would have improved things by giving the eggs longer to mature. My clinic altered my meds after this happened on my first cycle - I went from just being on Ovaleap to being on a small dose of Menopur as well, which contains some luteinizing hormone - my clinic said this helps by slowing things down a little in the hope that everything will then mature at a more even rate. No idea if this is done elsewhere, but it did slightly improve things for me (and I'm now pregnant after my 3rd round). Fingers crossed those 3 are all you will need x

Hi, so sorry that this happened to you. It’s so devastating when you get the result, especially when all along you think it’s all going to plan.

I had the same thing happen to me twice, lots of eggs collected and very few mature. I read about the potential of a dual trigger of HCG and Decapeptyl to increase maturation rate. I spoke to my clinic about it and at first they weren’t sure but they eventually allowed me to try it for a cycle. I ended up with 27 eggs collected and 26 ended up being mature!

Have a look into the dual trigger and maybe speak to your clinic about this?

I was going to suggest speaking to clinic about HCG trigger too - I think that usually makes a big difference

I am so sorry this is the siutation for you. I dont know if this will even help but thought id comment, I am 36 and my AMH was 13. I only had 11 follicles growing and 3 retrieved, 1 fertilised and obviously failed. Ive not had a meeting about it and no 1 has mentioned the low eggs- just said everyone and everytime is different.

I am currently looking at clinics in Prague and Spain for my next round, do you mind inboxing me the clinic you are at ? as language barrier is my main concern at this stage. any other hints/tips you have would be good.
