Hello everyone, we had our egg collection yesterday on our 4th cycle of ICSI, unfortunately I have very very low AMH and the only collected 4 eggs of which 3 were mature and today got the call this morning only 1 fertilised! The embryologist said it’s up to us if we have a 3 day transfer or 5 but she said if embryo doesn’t make it to day 5 in lab it wouldn’t make it in the body either ? Any advice do we wait or take the risk and have our only one transferred at day 3?

Hi..I’m 40 with low AMH. I had egg collection last Friday and managed to get 8 eggs. I received a phone call on the next day to say only one had fertilised. The embryologist suggested a three day transfer but would first see how the quality of the embryo was on day 3. After an awful weekend thinking that my one egg wouldn’t make it, I received the day 3 call to say that my embryo was developing well and at the stage it should be at 8 cells. I then decided to wait until day 5. Thankfully on day 5 I received good news that my one embryo was now a top grade 5AA blast. I’ve not had the transfer yet, it’s been frozen so I can do ERA testing. My advice would be to wait till day 3 and if it developing well wait till day 5 to have the transfer. I know it’s only one egg but that one egg can make it! I know how you feel as I was in the same position last Saturday, especially finding out one egg had fertilised. I just thought the worst and thought it wouldn’t even make it to day three. Aww good luck for the phone call on Monday and hope it’s good news that your little egg is developing well xx
Hi! I also have very low amh and only had 1 egg collected. I chose to take it to day 5 on my last cycle as I shared the same view as my embrologist that if it didn’t make it to day 5 in the lab that it more than likely wouldn’t in the body plus I wanted to do a frozen transfer as that’s where I have had success previously and all my fresh transfers have failed. Also I would rather have nothing to transfer than transfer an embryo that wouldn’t make it and go through the awful 2 week wait. Anyway it did pay off and I’m now almost 26 weeks pregnant. Only you can make that decision that you are comfortable with. Good luck with whatever decision you make xxx

Hey- I have low amh and only got 2 eggs in my round in Jan. Only 1 fertilised normally. I was meant to be going to day 5 to freeze. Spoke to the embryologist for an update on day 3 and she said it was looking average and couldn’t say whether it would make it to day 5. I couldn’t handle the thought of going through the whole round and having nothing to freeze or transfer so I asked to change to a day 3 transfer (had read that embryos can do better in your body than in a lab). They agreed and I had it put back in and got my first ever BFP. I am 20 weeks 5 days pregnant today. For comparison, my first round i had 4 eggs collected a fresh transfer of a day 5 4AB graded embryo which failed and a day 6 5BB frozen. If I go for IVF again I will go for a day 3 transfer as that is the only one that has worked for me.

I think you should do whatever you think is best for *your* mental health, since scientifically there is no definitive way to know whether a 3D embryo, or your 3D embryo specifically, would do better in the lab or in the uterus. Labs often think that if an embryo stops in the lab, that it would stop in the body, and that the lab may even be a *better* way to bring it to D5. Others say that the body might be a better environment. There is no evidence either way. So, it's really down to what you think will be easier on you. If you feel like you'll "what if" yourself to death if you choose D5 and it doesn't make it, or likewise if you choose D3...just do whatever feels right to you, since in all likelihood it does not matter either way and the outcome will be the same. xo
Thank you for your reply! What you say is so very true you just don’t know what is best as there really is no evidence to suggest either way! So I think all things considered if it’s going to work it will work at day 3 or 5 so all being well, assuming good news in the morning we will have our transfer tomorrow ! I have to say, this is fantastic advice. People should always follow their gut and what they believe. IVF is not an exact science and sometimes it is good to listen to all the advice and guidance and follow what you believe will make you most comfortable.
My clinic take the view that it's a better environment for the embryo in your body - last time I only had 2 embryos and they did a 2 day transfer with the best quality one. Personally I would much rather feel that I was in with a chance after going through all the pain of the egg collection, and transfer something, but I understand that everyone is different. I agree with the view that the best place for the embryo is in your body so I was happy to get it transferred asap!
There are also a lot of stories on here about embryos that have been classified 'low quality' by the clinic which are the one that works, when the better quality ones failed.

I'm in a similar position. I had egg collection on Friday and got 6 eggs. 5 were mature but only 1 fertilised. We're booked in for a day 3 transfer tomorrow afternoon but have to wait for call tomorrow morning to confirm how embryo is doing before going ahead with transfer. I've got the same fear that embryo won't make it but keeping everything crossed that it's a little fighter and that the call tomorrow will be good news.

I think I was going to say all that has been said previously which is go with your gut. You may want to wait to see that your embryo has a higher chance of implantation so wait till day 5 or you may want to not risk having no embryo to implant so then go with a day 3. Like someone said, the outcome is likely going to be the same.
