FET with endometriosis- what was your protocol? How much estrogens?

Happy New year everyone!

I was due to start my FET ivf treatment today ( we have one embryo from a first try ) but I've decided *not to go ahead* with my clinic protocol as I believe my estrogens levels will be too high for transfer day!

I've read new research saying that estrogen should ideally be taken less than 21 days and not more than 31 days before transfer for optimal results

Could I please have your exact protocol for an FET and the results? And whether your clinic is private or nhs?

Also, did you take gnrh agonist beforehand?

I feel my clinic ( nhs) doesn't really take into account my endometriosis :-( In fact the doctor said that the pill is a good treatment for endometriosis ( yeah right...) And I shouldn't worry. Buy I do..I'm almost 38...

Here is the protocol they wanted to put me on

Mycrogynon 30 for 24 days minimum ( this is to control my cycle)

Progynova for unknown days ( 1 to 2 weeks)

Scan to assess lining

1 week on progynova

Transfer

Progesterone until 12 weeks past pregnancy

Hey! Fellow endo-gal here. My last fresh transfer which was my first bfp, although short-lived, they suppressed me with Norethisterone for two weeks and then added in buserelin. Then during stim I took Progynova and then progesterone. Def think it is important they down-regulate your system somehow as there have been studies which you have maybe seen which suggest (although the jury is out, as usual) endo patients benefit from that but yeah, not sure about the month of micro. When you get down to it though, I think the most important thing is that you trust in your doctor so if this protocol doesn’t feel right to you, ask to discuss your options. Because you do have them, nhs or not! Good luck! Xx

I had a failed FET before Christmas and my protocol was buserelin injections to downreg. After down reg confirmed, progynova for 12 days with cyclogest 5 days before transfer. I was on three progynova tablets per day (starting at 1 and building up) and four cyclogest pessaries. The buserelin was 0.5ml and I had to continue that until I started the cyclogest. I'm downregging now for another round of stims (I only had 1 embryo) and they've changed my protocol for this bit but I think they'll keep the FET protocol the same.

How long were you on buserelin?

4 weeks I think. 2 weeks until I started progynova and then another two weeks until I started cyclogest.

Hi

I can't remember exact protocol. I down regged on buserlin, then progynova (which is oestrogen) and then transfer. Apparently they would have kept me on progynova and cyclogest for 12 weeks up to the first scan.

Sadly bfn. Private treatment.

One thing, I'm on a fresh cycle and microgynoned for two months before. It did suppress a cyst on my ovary (seen before the pill and not afterwards) and the period wasn't as bad as the cycle without the pill. Surprised me

Nhs put me on prostap for 3 months before they started treatment they recommend this is good to shut down the endo before they give treatment to make success rates higher. So you have high esteogen? I think I have high estrogen. What are the symptoms? xxxx

Estrogen dominance is often common in those with endometriosis and the last thing you want is to get unnecessary amount in out body.

This might explain why I feel so crap over a week before my period is due. Do you think this is worth mentioning to my dr. I get really sore boobs after ovulation before periods is that a symptom? I’ve got stage 4 endo xxxx

That's the exact plan my private clinic has put me on too for FET I'm due to start it in 4 weeks. I have severe endo. My first round of IVF worked and 1 year later I'm trying my frozen embryos, I just wanted to say good luck :)

I’m in the US, so it’s all private and every clinic makes up their own rules. I’ve done four FETs, four years after success in my only IVF fresh cycle. First one failed due to a hydrosalpynx and then I had surgery to remove it and diagnose endometriosis.

All of my FET cycles have been the same - at least two weeks on the pill (Nortrel, and this is where they adjust days for whenever there’s an open transfer time at the embryology lab). Twelve days of Estrace (2mg three times a day), followed by an ultrasound for lining check. Mine is always around 12mm at this point. Then I do a 1cc progesterone in oil injection timed with the transfer. From there it’s five days on the same dosage of Estrace and PiO, then the transfer. They keep you on the same protocol until 10 weeks of gestation.
