see your GP. It could be that your cervix is causing the spotting, when was your last smear and was that normal?
Even when spotting, the uterus is contracting and this causes pain if there is endometriosis or adenomyosis. However, spotting itself can be caused by the cervix and you should get it checked out either way. Infection generally can also cause spotting, infection of the vagina, cervix or uterus. Is it foul smelling or is there a fever or general symptoms of illness present?
As you say Gynae are clear it’s not the cervix itself then your query is it endo - these are known symptoms . You might want to be querying DIE in the pouch of douglas area. Lindle has a number of very clear posts about how to ensure you get the best imaging to hopefully show that.
