Know your Pancreas Beta cell Status to assess the progression of your Diabetic conditions & the need for Insulin: Type 2/ Type 1.5/ Type 1

This is further to my post on Insulin Resistance.... which if you have missed, I would request you to please go through the same which I posted few days back .

Today we are going to check and diagnose our diabetic status with specific reference to our pancreas Beta cells which many medical professionals tend to ignore, though pancreas beta cells are crucial for our diabetic conditions ...

Once you are diagnosed with Type -2 diabetes, you consult a Diabetelogist/ Endocrinologist.

Based upon your Fasting/ PP Blood Sugar, HBA1c, Your Doctor puts you on Oral diabetic drugs. You are told to do physical exercise, control your diet, Reduce Carbohydrate , Fat and so on.....and thus the life starts exploring ..........one keeps experimenting with different Diet , Physical exercise, bringing changes to the life style.... Looking for alternate medicines in Ayurvedic , Homeo pathic , unani and many other remedies suggested / advised by many medical / nutritional experts who basically target reducing blood sugar.

But unfortunately, in most cases, the underlying conditions keep lingering which only flares up at a later date to cause diabetic complications and could lead to injecting exogenous insulin ultimately .

Mr. Cyrus Khambatta, MD, Ph.D in Nutrition Biochemistry, the Author of the Ny Times Best Seller book "Mastering Diabetes"(who lives with type 1 Diabetes for 40 years since his childhood) writes that a simple Blood test called the C- Peptide test at the time of diagnosis or at any stage of your diabetes would indicate how much Insulin your pancreas beta cells are capable of making and could decide the course of your medical treatment.

1. If C-Peptide value exceeds 4 ng/mL, your pancreas are in overdrive to secrete excess Insulin. You have a very strong Insulin Resistance and if you are able to enhance your insulin Sensitivity by a suitable diet, exercise, life style changes , you have done it. You don't need medication at all. However, if you don't treat your IR- Insulin resistance, it can create a host of issues and your IR can damage your pancreas beta cells ultimately which can force you to take injectable insulin.

Excessive insulin is normally associated with obesity as is commonly found among Caucasians, Africans and Hispano's, though nowadays it is found among Asians too to some extent.

The only way is to monitor your IR- Insulin Resistance with a suitable diet and exercise , bring in lifestyle changes to sustain weight loss with an objective of meeting the 7 point checklist as mentioned in my previous post under Insulin Resistance .

2. If C-Peptide value between 3 to 4 ng/mL would indicate that your Beta cells are capable of making sufficient Insulin, but you have a moderate Insulin Resistance. You too can reverse your IR-Insulin resistance by a suitable diet ,exercise and life style changes , you may not need a medication at all.

3. If C-Peptide value between 1.5 to 3.0 mg/ mL would indicate that your pancreas still makes adequate insulin and you have some mild IR-Insulin resistance.. which you can reverse by maximising your insulin sensitivity by proper diet , exercise and life style changes .

4. If C-Peptide value less than 1.5 mg/ ml means that your beta cells are damaged to some extent. . Unfortunately Damaged beta cells are difficult to repair...It may be difficult to control your blood sugar by Diet and exercise. Insulin is a master hormone.. it is required for metabolism of any diet you take whether it is carbo , protein or fat dominated. So if insulin production is inadequate , then you would struggle to control your blood sugar whatever food you take.

Oral medications would be of some help at early stages .... You can keep trying the alternatives in allopathy along with suitable diet & exercise ......Ultimately , injectable insulin may be required sooner or later and it may be wiser to opt for insulin sooner than later in stead of trying out different medications which could result in to health complications.

These kind of type 2's are normally thin built body with a lower BMI as commonly found among Asians, though nowadays prevalent in other continents too. So eating a healthy diet along with physical exercise , resistance training to gain some weight to reach your recommended weight / BMI would be helpful for your turn-around.

Mr Cyrus Khambatta recommends a "Diabetic Antibody panel" test for these class of people to rule out "LADA" - as described in next Paragraph.

5. If C-Peptide value between 0.2 to 1.5 ng/ ml may indicate that you may be Type 1.5 also known as LADA ( Latent Autoimmune Diabetes in Adults ) . If you are above 30 years age, you should go for a " Diabetes Antibody Panel" test which would indicate LADA if you test positive for one antibody. Presence of antibody would attack your own beta cells and ultimately you will become insulin dependent ....you may have unexplained weight loss, frequent urination , muscle cramp , low energy ... Excessive thirst...

At times Type 1.5's are mis-diagnosed as type 2's. If your C-Peptide value is less than 2 ng/ml, Mr Cyrus khambatta recommends a "Diabetes Antibody Panel" test to rule out Type 1.5 or LADA. The earlier LADA is diagnosed , the better is the outcome of treatment .

6. C-Peptide value less than 0.2 ng/ ml would indicate type 1..on testing diabetic antibody panel , you may test positive for multiple antibodies.. you may be younger than 30 years.. ...and you may have all the symptoms of type 1.5...you become insulin dependent.....

One thing good is that diabetes is not the end of life. People with diabetes live longer whether Insulin dependent or Insulin independent ( IDDM / IIDDM) ..because they learn the hard way as to how to live longer with suitable changes in diet, life style and exercise ....

On the c-peptide value, does it mean 1.5-3 mg/dL is the best range?

If C-Peptide value is 1.5 to 3 ng/ml, the prognosis is good...

You can manage without any medicines whatsoever by keeping the IR in check..

You can follow your lifestyle conditions and a diet such that all the 7 check points mentioned in the post " Insulin resistance " are met without any medication...

And I strongly believe it is possible !🙂👍 Keep it up !🙏

Interesting. so, when a person has high BS levels, the first step should be to check the underlying cause behind this i. e. to check the insulin levels in body, then the treatment should start. But, how many doctors do this? As far as I know, doctors just check Fasting, PP and a1c and start medicines to lower BS and by trail and error methods of few medicines for few months, one or two medicines are finally settled. No proper dietary guidance is given, just asked to avoid sugar and sweet fruits and lower rice and potatoes. This is condition of MDs in India, with very few exceptions.

I agree ...

There are very few doctors who would recommend a C- peptide test on their own.....

That is why you should insist for a test ...and in all probability he may agree..

He may also tell you that you don't need it.. in such a situation you should go ahead and order a test on your own.. there are lot many on line diagnostics center in India who would not ask for a doctor prescription slip and they will do the test for you...

Metropoli and a lot of other diagnostic centers also will not bother to ask you for doctors recommendation... At best some may ask you your doctor 's name . .. which you can always tell a name..🙂

At least by the test result you come to know your pancreas beta cell Status and where do you stand and what should be your way forward...and plan your own way forward. ....after all life is yours !👍

but india is poor country and c-peptide antobody etc mght be costly tests so thats how this mechanism would have developed

C-Peptide is a common test costing just Rupees 1000. It is a very important test... you would come to know where do you stand with reference to your pancreas beta cell Status.. you can get it done during your next routine blood sugar , HBA1c test....

You need not worry if it is around 2ng/ ml...

If it is less than 1.5 ng/ ml, you need to consult your endocrinologist/ Diabetelogist and ask him if he would recommend for an diabetic antibody test. There are basically 4 antibodies tests.. your doctor will decide what to order...

(1) GAD65 Igg

(2) Islet cell cytoplasmic autoantibodies (ICAs)

(3) Insulinoma-associated-2 autoantibodies (IA-2As)

(4) Insulin autoantibodies (IAAs), which are more common in children than Adults.

The first one is costly.... may cost around Rs 6000 which is commonly prescribed.

Agree ...these 7 points check list are strong indicators for insulin resistance...If you have incorporated some life style changes, physical exercise, dietary changes ....these 7 points check list would indicate how effective are these at a given point of time and whether you are advancing towards reversing your Insulin Resistance !

With regards to dietary pattern, it is a complex subject ....people need to choose their own diet what suits him in order to meet the 7 point check list for insulin resistance which I have taken from - with courtesy the author Cyrus khambatta ...MD, PH.D.

On diet ...what suits you may or may not suit me... What suits me may or may not suit you..

So , I would not speculate....nor urge someone to change his current dietary practice if he is already doing well.....

However , I would always refer evidence based long term scientific studies / literatures on long term health benefits ...

May be one of these days I would like to post for information and reference ....so that members can take informed decisions ....

Nice post... I really wonder why our doctors do not tell us the truth...

my c-peptide reading was 3.91 and I have asked about this to my physician... He informed that you are Insulin resistant.. . but my pancreas producing the sufficient insulin.. but body is not able to use.. upto this is fine... then he prescribed for insulin injection... then my question was that since i am resistant to insulin, then what is the use of injecting it again?? then his answer is that the insulin produced by body is not having quality... thats it... I am still wonder what is this quality means???

However, you are right that by reducing the insulin resistance by exercise and diet I do not required to take medicines..

You are right in your thinking.

The doctor was trying to fool you.! 😀

How can external synthetic insulin could be of better quality than natural insulin produced by human body?

It is like telling milk powder supplement is better than mother's milk for the baby.

Your pancreas is in excellent condition and you have insulin resistance for which you don't need medication...and with your condition , I strongly feel you can reverse your insulin resistance by diet and physical exercise.

First try with a calori restriction or even Intermediate fasting. .for example , take your dinner by 6 PM and next day breakfast by say 9 AM....this becomes 15 hours fasting... Which you can manage as long as you are not underweight...

There are members who take one meal a day to fight insulin Resistance successfully .

However , if you can not do fasting , then my strong recommendation would be to buy a treadmill for your home... Post every meal, please do a treadmill walk for 25-30 mnts ...

But your diet is also important. You may Please do read my recent post on what to eat !

Are there any different parameters for fasting c peptide test results and post meals test? If yes, please inform the both.

Or it do not effect like hba1c test?

To measure level of c-peptide a fasting blood test is taken.

You will be asked not to eat or drink (certain fluids) for 8 to 12 hours before the test.

HbA1c test, blood can be taken any time of the day or night!! This is because the test is looking for 90 day average.

If your c-peptide comes too low, you may have to consult your doctor for further antibody test to rule out if you are LADA or Type 1.5...

LADA or Type 1.5 people are normally lean and thin. Perhaps underweight...
