I think at this point you need to have a talk with your doctor about changing your medication. The one you're on is great for depression but not so much for anxiety, which you have by the boat loads. It's not uncommon to have to try several before you find one that works with your body's chemistry. Just breathe. You're going to be fine, you just have to get through this rough patch.
