Major anxiety over a woman I love very much.

So I am having anxiety issues worse than ever. I have had anxiety before but never like this. There is a woman that I am deeply in love with and to give a little back story on this situation, we dated a few years ago after being very good friends. It didn't work out but we remained friends. Now we talk somewhat regularly and make sure we set aside time so we can see one another from time to time. I know this isn't a love forum and anxiety is a huge part of love but it is still eating me alive. I can't sleep and its affecting me in my daily life. I have a newborn nephew that I love very much but for some reason I don't care if I see him atm and I hate feeling that. Things that I love doing don't seem fun or interesting all the sudden. Also I am about to tell her how i really feel when i see her again while also unloading a huge secret onto her that I have kept to myself for almost 20 years. I just dont know what to do and the thought of her not loving me at all in the way I love her is eating me alive. Please help with any advice.
