I'm new here: Pain and Panic Attacks

History of mild to moderate anxiety, but recently I have developed tension headaches (in the back of the head), and sometimes it migrates to the neck/shoulder/back. First bout lasted about 2 weeks before I "cured" myself through will and healthy habits (or so I thought). They come back occasionally and last few days have been worse. The pain is "mild" but the sense of unease that comes with it is severe. I also have times where I wake up right after falling asleep thinking I'm about to die for no good reason. Sometimes I can ignore and go back to sleep but I think it was a full on panic attack last night. It really felt like I was going to die. I think about all my pains and symptoms and can only conclude that I'm broken. I'm 29.

In general I take better care of myself that most, because of my health anxieties, meaning I go for long walks outside and eat healthier than most Americans. I'm willing to give up gluten, dairy, whatever it takes to beat this thing, but I'm worried diet changes won't be enough. I need help.
