Enzalutamide instead of Orgovyx, as medical treatment?Anyone tried that?

My husband is still being worked up for cardiac issues, due to growing Aortic Root Aneurysm , unstable BP ( while on Losatan and Metoprolol) and moderate-severe Aortic Valve Regurgitation.

He stopped Abiraterone/Prednisone in mid December 2024, due to concerns with cardiac toxicity. He had been on Abiraterone for a year.

He has been on Orgovyx since October 2023.

From what I have read, Enzalutamide is less cardio toxic than Orgovyx. Is it an accepted protocol to be just on an Androgen Receptor Inhibitor?

Husband is 74 and was diagnosed with PSA 0f 46, and Gleason 9,in 2023, localized, but assumed systemic.

He had 28 Proton Radiation treatments in Fall of 2023. He has Radiation Proctitis with fairly regular episodes of rectal bleeding ( 6 times in January so far)...He is mildly anemic, low hemoglobin.His gastro doctor just cancelled a sigmoidoscopy due to concerns re heart. He was supposed to get a second APC treatment.

Also has been on daily antibiotics for at least 2 years for resistant bacterial prostatitis, ( Kliebsiella Pneumonia)

Thoughts on just using Enza? Any other PCA treatment protocols that can protect the heart?

2 yrs of antibiotics for prostatitis?? There are bacteria in the urinary tract that are innocuous in most men. Some of those bacteria can cause problems but usually don't. It's like Staph aureus that colonize the skin but don't usually cause an infection. Prostatitis is almost always relapsing and remitting.

I appreciate that Monotherapy will not be as effective as a combination.

I should have been more specific. He has been off and on antibiotics for the past two years, mostly on. During the two years of antibiotic treatment after completing a recommended course of treatment( 4 weeks, 3 months, six months), he would go off the antibiotics, then within a week to 10 days he would get a raging UTI, once to the point that he had to be catheterized. He did consult with an infectious disease doctor at Mayo. Currently his urologist has recommended a half dose of bactrim .His infection is resistant to multiple antibiotics. It has been complicated.

Your husband should see a hematologist who can prescribe iron infusions to help eliminate the anemia. I had to do that after many episodes of radiation cystitis and bleeding. Also should see about hyperbaric oxygen therapy to eliminate the proctitis and associated bleeding. It is a proven therapy that might help. Maybe see a gastroenterologist for guidance on that or call a wound care center (often part of a hospital) to see if they have a hyperbaric chamber.

Thanks for your recommendations. He is only mildly anemic (slightly low hemoglobin), and his iron is low normal. He does need to get more specific advice from his MO and Gastro Doc about iron supplementation. He has messaged his gastro doc again.

Hyperbaric oxygen therapy is something we should explore.

I used Enzalatumide from 2018 until 2024. It worked for me pretty good , kept PSA low for a long time. Instead of the prescribed 160 mgr ( 4 pills) , my oncologist recommended 3 pills .

Side effects: fatigue ( but a daily nap took care of that ) and it partially blocked my oxycodon from working properly . Had to take a slightly higher dose.

Psa starting rising. Oncologist saw no reason to continue. Had Cabizetaxel chemo , psa down again.

How many cabazitaxel infusions do you plan to have? Do you already have in mind what your next treatment might be?

Had 10 , three weeks apart. From may till the end of November 24.

Next treatment will be Lutetium 177.

I must say I stuffed myself with fistful of supplements. Aspirin was the first . I almost got in trouble with that and NIH almost booted me out of the Xtandi trial. There are numerous screeds in here about my ordeal. I’m not recommending anyone to do what I did. I’ll never know if any of them helped. I took 3 Xtandi for years , recently my calcium went up so I started on 4 a day. Calcium went down. I’ve stopped all supplements a year ago. Read my other missives to get a sense of what I did. I’m still on three antidepressants, one is desimpramine shown to have an effect on stage 3 but I take it anyway.

Supplements can be a money pit , and aren’t all benign … I remember being prescribed a lot of different supplements many years ago by a naturopath and ended up with double vision , had to go off every thing.

My husband takes many supplements as well… not clear if they actually are helping

Enzo can work along with a natural approach as it has done for me. DM me as they attack me on here. I have a bovine aortic valve and full ADT pulled on my EF fraction 25% so be careful.
