Dear All

can anyone help. Diagnosed with stage 1V advanced metastatic prostate cancer December 2022. 6 sessions of Chemo, then Firmagon and Nubeqa PSA was 349 now about 1 but has been increasing. Cancer was in most bones but no organs - primarily pelvis and hips - have had radiation treatment but still suffering from pain at night when sleeping on my back in the pelvis area - just cant shake of the pain - any tips on additional treatment or medication - thanks clive

When PSA doubles, you can do Xofigo with (less) docetaxel -perhaps that will help with the pain.

I use THC for sleep and some pain and Psilocybin for metastatic

morphine helps me sleep just 15 mg brings the pain level down enough that i can find a comfortable position and get some sleep.

how many drops is 15 mg or are you taking tablets

I am sorry you’re going through this. Pain is a bugger and a major part of the prostate cancer journey. I have been stage 4 since December of 2018, and I have been through countless treatments currently doing Pluvicto Lu 177, I have my 4th TX on 3-21. Which of all my treatments seems to be the most effective. My pain medication regimen is Fentanyl patches, long acting Oxicotin, fast acting hydramorphon, along with a number of other meds we have tried. As you Know this is a roller coaster, with us all having the same ending. Ask your oncologist to get you a Palliative care doc. There is also a pain pump which a lot of men are having success with. The doctors do a good job of maintaining my pain but it starts with me asking and communicating what you want or need to deal with the pain. Do not fear addiction issues, at this point it is no longer a concern as I have already stated, we all are ending at the same place. Good luck attitude is a big part of this journey, stay positive and remember they are coming up with new stuff constantly. I’m on meds now that were at the clinical side when I started this. I’m a longtime member of a 12 step program and understand. It’s easy for people to say you’re dying anyway, don’t worry about it etc but it’s not that simple. Do you have a history abusing opiates?

Hi Nyon. I have a similar problem. You can try changing your sleeping position. Try sleeping in a lazy boy or on a comfortable chair or sofa. Try sleeping in bed on your side with a very firm pillow between your knees. Surprising the amount of pain relief you can achieve by finding the best sleep position for you. I tried Zomorph 10mg caps but were useless. Dr may up the dose later. I also tried Zopiclone 3.75mg. It lets me get around 3 hrs decent sleep which I can manage on. Prior to that I slept every 2 or 3 days which wasn't enough. Hope I have been a help.

I understand your pain. I have chronic low back pain (fused L3-4-5, getting ready for my 5th surgery to fix L-2 level). I take 75 mg of morphine (mostly extended release) and I have a high quality of life. Need to find a pain specialist doctor that will prescribe high dose morphine. You're addicted. But, what the hell, if it takes care of your pain, it's worth it.

try 10-15mg of THC gummies, or smoke a number 30 minutes before bed…I also microdose 250mg of psylocybin combined with50mg of CBD

I had miserable pain from radiation that blasted a nerve near my spine. One thing that helped some was sleeping in a recliner. 2100 mg of gabapentin/day also helped. The pain mostly resolved in a year but, looking forward, I got a bed that rises up like a hospital bed. I hope you find relief.
