I just added more content to my bio regarding my husband's history and current status. I'm highlighting the current treatment for those who might be interested:

5/2023 PSA of 17.1 (holding steady near that reading for the last three tests)

He has been on Lupron since PR and subsequent localized radiation in 2014. When it started to fail a year ago they added Xtandi. Now that is no longer working and have completed 5 of 6 treatments of Xofigo - which he is tolerating well (blood counts are holding up, with only minor indigestion/diarrhea. The most likely next step as things progress will be chemo (docetaxel). Continues to be asymptomatic - no actual pain from the disease itself.

Pluvicto is another alternative. Fewer side effects than chemo.

Novartis tries to get Pluvicto approved for use before chemo. This is not done yet.

Our MO hasn't had a lot of experience with it, but we'll address it with him.

Then I guess he will recommend chemo. Did you get Xofigo from a nuclear doc? He will know more about Pluvicto.

Consider adding back Xtandi when he has docetaxel:

Does one need to be off of Xtandi for a time before starting it again? He's actually still taking it, and is a bit concerned what might happen if he takes a break, assuming it's still doing something.

In that trial, it was no longer working. Adding docetaxel gave it new life.

Appreciate your thoughts. I’m about to start Xofigo and MO wants to stop AR ( Erleada) . Similar to Tinuriels husband, I am concerned about stopping an ADT component of my treatment. Your thoughts?

As long as he is taking a bone strengthening agent (Zometa or Xgeva) with it, it shouldn't be a problem.

Hope this helps- My husband is one week into his 2nd docetaxel infusion. He has minimal side effects. To my surprise, he feels better, especially after his second infusion. His PSA came down & his next blood draw is in 2 weeks. I met another man in the lobby who was on his 4th infusion, he too had no complaints. Everyone is different, but many men tolerate it well. So far, we’re very pleased. Best
