I had a Gleason 4 tumor (2 cm x 1 cm) with a PSA in the 20's. Is this saying that targeted treatment with no or limited ADT would have been sufficient, especially if HDR brachytherapy were used with IMRT?

Nope. It's just saying that results of external beam radiation (EBRT) are improved with a little extra boost dose to any MRI-detected tumors in the prostate. (It confirms the FLAME RCT.)

It doesn't address ADT (everyone in the trial had at least some ADT), which others have shown is required for 27 months when only EBRT is used for high-risk patients.
