This is one of my very few posts. I've been reading these posts from my brothers (and sisters) for several years and think they've help me survive this weird affliction.
History: In Dec 2018, diagnosed with CRPC after an MRI for a back problem showed signs of metastasized PC. Went for labs and had a PSA of 1700. Local Oncologist started me on Lupron in Jan 2019. I thought I should follow family members who had been treated at Memorial Sloan Kettering with good results, so for the past three years have seen Dr. McHugh at MSK. Their team, which I think is terrific, added Zytiga in Feb 2019. Bottom line is that I seemed stable for about 3 years with fluctuating PSA from 7 to 11, stability verified with bone and CT scans.
In July 2020 The radiologists noticed a small Lung nodule (<1.0cm), which was too small to biopsy. Since I live in FL, I often get local support from Moffitt, (flying to NYC area for a Lupron shot is very costly) so I sometimes deal with slightly differing opinions. As this spot has finally reached 1.0cm I asked Dr M about a PET/FDG scan. prior to an invasive biopsy. The Nodule lit up handily, making the biopsy a good idea. The lab analyzed the sample as an Adenocarcinoma, - and a Primary Lung Cancer. I have a Telemedicine call scheduled 9/30 with one of the Lung specialists I/R Doc's to discuss possibilities. Any suggestions from this group would be appreciated.

A similar thing happened to someone in one of my PCa support groups. Because there was only one spot of lung cancer, they were able to zap it with SBRT-- 54 Gy in three 18 Gy fractions. SBRT gives equivalent results to surgery and is less invasive and has fewer side effects.
In a way, you were lucky to find it while it was still stage 1. Unlike prostate cancer, there are no biomarkers and no symptoms until it has metastasized. Fortunately, the cure rates are almost 100% at this stage. My friend got annual CT scans for 3 years after SBRT to double check that they got it all.
Had SBRT at MSKCC in Commack this past week. 3days, fractions of 1800GY total of 5400. They believe with periodic CT scans i've got a high expectation of cure. I guess that leaves me with my plain old mCRPC Thankfully still responding to Lupron and Zytiga. Having no pre-knowledge of SBRT, this was a fascinating procedure, I never thought being able to hold my breath for over 2 minutes could aid my treatment. We learn.
I smoked until 23 years ago. There was immediate family who had different varieties of Lung Cancer. My Mother, a heavy smoker died of it many years ago, My Brother had a top lobe, right lung removed and stayed clear for 6 years. Since he was technically cured, He decided to follow me and acquire Prostate Cancer. He passed last year from unrelated issues.
