Has anyone used this combination? My oncologist is thinking of this. I'm th liking of Pluvito. Had PMSA scan that showed multiple Mets on spine , ribs, pelvis and femurs.

Do you know the SUV values of the mets? Did you get a FDG PET/CT. ?
If the SUV values of the mets are below the SUV value of the liver the response to Lu 177 PSMA treatment will not be optimal.
Have you discuss having a biopsy of the mets?

I just re read my Pylarify Scan Notes, and there is no mention of SUV in Liver, only the 3 Lymph mets they found. SUV of 6,12,17. So cannot compare to Liver?

12 and 17 are above the SUV of the liver which usually has a SUV around 7 to 9. I believe you could have qualified for the Vision trial.

What advantages or disadvantages, including price, does it have compared to other agonists?
same as Lupron or Eligard
it was explained to be newer option. Same results but half the medication of Lupron and Eligard. Therefor not as much to process with side effects. And Lupron was in short supply over the last 2 years.
It's possible, but I wasn't told that. My first shot was Trelstar four years ago, but by next shot I had moved over to a different institution in a different city, which used Lupron. My understanding at the time is that they were essentially identical (as opposed, say, to Lupron vs Firmagan), and that each institution tended to align themselves with one drug or the other, for whatever reason. But I certainly didn't drill down on that at the time. Along the way, when there was a Lupron shortage in 2020, I got 4-5 Eligard shots, then went back to Lupron in 2021. I haven't noticed any difference in side effects, but most of that four years I've had Lupron.
What was your PSA at the time of the PSMA PET scan?
Do you have bone pain?
PSA was 4.5. I have some pain in hips and spine. Been Advil Dual makes that manageable.

that is very important if you want to do Xofigo. My medical oncologist said that Xofigo is only indicated for bone pain paliation but it will not fight prostate cancer. Chemotherapy would be better or if you have only pain at one site than maybe that site could be radiated if the chemo will not help with your pain.
You could save Xofigo for the future. It can break bones if you are unlucky. I am not a doctor nor i am in a position to see everything but i wouldn't jump into Xofigo if I could solve the limited bone pain by other means. Xofigo is not risk free.

you can only receive 6 Pluvicto infusions in your lifetime because of the radiation.
If you would fill out your profile better maybe we could advise you better. It is very risky to say anything if we don't have your full information. Pluvicto is usually given if your life expectancy is under 8 to 10 years. We don't know your age etc.
a bone strengthening program with either Xgeva or Zometa will help reduce risk of adverse skeletal events (fractures etc). Combining with celecoxib May improve progression free survival.
PSMA PET scan can show if Pluvicto can be of benefit vs Xofigo now. With either of these try to get Provenge along with either Pluvicto or Xofigo for enhanced immunologic response.
These are just some ideas to consider and discuss with your oncologist. Not prescribing. Details of your status must be all considered.
