Genomic testing of the primary tumor, or a metastasis? What Gens or ARN or what is to be tested ?

I am mCRPC

my urologist wants to biopsy the prostate to have data for immunotherapy if it suits me. Pembrolizumab, Olaparib, whatever

I have read about TILs, and wonder if anywhere it is available, as if I am biopsied, there will be samples to work with, eventually.

Is there a list of Genes, ADN, whatever, that is mandatory to be tested ? I wouldn't be happy to know in a few weeks that the biopsy was done, the testing was done, but some testing was not done.

And if TILs therapy is available anywhere, this would be a good time to try to find some TIL cells and harvest them and then multiply them...

What is TILS?

A biopsy of the prostate tissue tell you if there is MSI-hi/dMMR (which would make you a candidate for immune checkpoint blockade) or BRCA2+ (which would make you a candidate for a PARP inhibitor). It is possible that BRCA2+ will only show up in later metastases.

The company doing the genomic analysis can provide a full list of the genomics they look for. Those are the only ones that are actionable now.

Might TILS be tumor infiltrating lymphocytes? Don't think a genomic test will show that.

It seems that you had a biopsy in 2019. Then you can still use the samples from the biopsy for genetic testing.
