ALT (SGPT) - 5X normal limits - Concerning? Mets to liver lungs and bones

Hello everyone,

Just want to give an update on my dad (69) has mets to the bones, liver, lungs and pelvic lymph nodes, PSA 113, Gleason 4+5 upon diagnosis back in July 2021. He started Firmagon and then on Zoladex and then 6 cycles of docetaxel which finished at the end of November. His mets in general have reduced significantly in both the liver and lungs.

His latest PSA test in April 2022 is showing 0.22, doubling about every month. Recent scans performed show that no new lesions and stable however his ALT(SGPT) was 32U/L in November 2021 and now showing 108 U/L (5x normal range as per the lab) in April 2022.

Currently he is only on Zoladex and no new meds introduced since November 2021. He does not drink alcohol. What does this mean? Liver damage from the mets? the ADT? Will this be a bigger issue when he eventually needs Zytiga?

Could be from the metastases, from the destruction of the metastases, and/or from chemo .Hopefully, it will come down over time.

This is secondhand info but I hope you find it useful. First liver tests go high, then fatty liver, then nodules on the liver, then possibly cirrhosis. Even cirrhosis is measured in stages, just like cancer.

Don't freak out just because liver tests are high. Get additional testing to see what's going on with the liver. Meanwhile definitely try to reverse the trend.
