Fluctuating psa with xtandi

My father has been on xtandi for 2 years and over the last few months his psa was rising steadily we assumed it was starting to fail however his last 3 appointments denoted slow steadily declines in his psa. Now oncologist wants to remain on xtandi and review in 6 weeks. Any insights or wisdom as to why this would be occurring? Thought this could be useful to share.

It would be hard to say without knowing what changes occurred in his eating and lifestyle. But enjoy the decrease in PSA and stay on xtandi.

Thanks yes trying to get the most out of it. He takes a heap of other supplements but wasn’t a change in regimen. I thought it was odd.

PSA can be erratic. The only sure sign that it's failing is if scans show more/bigger metastases.

Thanks yep I knew it can jump around but it really did go up consistently. Scans have remained stable.

You could consider getting scans. If you’re not metastatic, you might be able to use those three consecutive psa scores to get on daralutimide.
