Testosterone lowering but PSA rising

2015 Prostatectomy, external beam radiation and two yrs ADT (Zoladex) and four yrs active surveillance. However, Dec 2020 PSA was 2.59 and Dec 2021 now 6.9. Testosterone Dec 2020 was 22.2 but Dec 2021 it has dropped to 13.6. Why is my testosterone going down while my PSA is rising? I understand the rising PSA but why the dropping Testosterone? Are PC cells ‘consuming’ testosterone and thus lowering it?

Those are in nmol/L I assume. If so, it is all within normal range. T fluctuates - nothing to be concerned about.

Thank you for your replies. Yes, my T is measured in nmol/L and PSA in ug/L. My T is now the lowest it has been since the ADT knocked it down to its nadir (0.6) in July 2017. My PSA doubled from Apr of this yr to Oct (2.70 to 6.01). As of last week my PSA was 6.90. While the T is within Normal Range (5.2-23.7) its continuous decline combined with a steady PSA is a tad worrisome. My first PSMA PET in May 2020, followed by two internal e-coil MRIs detected a single lesion/tumour (9mm x 9mm x 2.3 cm) quietly residing where my right SV used to be. I realize that even PET scans have their detection limitations (ie can't detect microscopic PC). I have a second PSMA PET scheduled for first week in Jan and then I will be starting degarelix. I'll talk to my Oncologist regarding a 2nd Generation therapy as well.

Hi there. My path has been similar to yours with the addition of chemotherapy. I have been on Zoladex since 2019 but this year my psa began to rise from 0.3 to 0.8 then 1.7 at which time my oncologist added Xtandi to my treatment. My psa dropped back to 0.3 after three months which, I think, surprised the oncologist.
