Husbands ALT/AST has gone high again after 2 years on Zytiga , 3.5 years on ADT and Prednisone , PSA is great 0.1 . Blood draw again on Tuesday and if it is still high we want to discuss taking 2 with high fat break fast verses 4 on empty stomach. My question is, would 2 lower his liver tests ? Any info is appreciated. Thank you.

Its worth trying to take 250 mg of Zytiga with high fat breakfast. And then check AST,ALT and bilirubin again after 4 weeks.
No, that will definitely not lower his liver enzymes. Taking two with a high-fat breakfast gives the same absorbed amount of abiraterone as four without food. Fat increases absorption. It is just a way to save money. Because the exact same amount of abiraterone gets into the blood and goes through the liver it does not cut down on side effects.
Now that cheaper generics are available, it is better to know exactly what dose is being absorbed by taking it without food.
As TA said you don't want to take Zytiga with food. Your ALT & AST are ok to go up to 5 times normal. How high are Hubbys? When mine were just about that high I stopped Zytiga. It took about 7 weeks to get liver numbers back to normal. Started back up with 1/2 dose(500 mg) After my liver numbers remained normal I went to 750mg. Been there for about 30+ months and PSA is <0.01. Good luck to you and Hubby.
I would suggest taking a break from Zytiga and wait till liver normalizes. Resume with 500mg and watch ast/alt. I wouldn't fool around with taking low dose w/food at this time. I've been on low dose for 30 months.
I have taken Zytiga for six years until recently, but always followed the full dose and on empty stomach. Most of my blood tests have been outside boundaries. But my feeling is that if I am not feeling too good, the cancer most have a really hard time. And then I continue the medication.

Thank you for the responses his ALT was51 and AST 74 . It is being retested today.
