I am 17 years in the battle after surgery failed to cure me, Gleason 8, no mets at the time of Surgery. In the past 17 years, I threw every drug in the book at it, including some treatment with Dr Charles E Myers (Leukine, Keto, and estrogen patches) and that gave me a very long remission after Lupron failed and other hormone drugs failed. Earlier this year, completed Radium 223 but 1 month after my final injection, my PSA started running amuck very fast and got up to 22. Still had some pelvic pain so I started Chemo. PSA slowly dropped to 6 and pain went away. Just did my 5th round of Chemo, but not sure what to plan next in the battle.
Need to plan now because my 6th round of the final Chemo is in 3 weeks.
1. Have a met on a rib that is a little painful and thinking about doing radiation on it.
2. Chemo doctor says if my PSA keeps dropping, to do another 2 rounds of Chemo, but side effects are starting to get worse.
3. Thinking about a clinical trial at UC Davis. There is a clinical trial running LYNPARZA (Olaparib). They can take a biopsy during and after the trial of that painful rib. I dont have to have the genetic match for optimal effectiveness of Olaparib, but to my surprise, Olaparib will be given in this trial even if there is not a genetic match. May even get Cediranib. (Cediranib constricts blood vessels to tumors to prevent tumor growth).
4. Forum says a restart of XTANDI may be possible.
5. Somebody in the forum suggested some anti cancer vitamins but I may not have much time to try this before I have to go back to traditional treatments.
Dont know what other treatments are out there so I hope the Forum may have some ideas. Appreciate you comments.

I think you could try too more sessions of Chemo and decide afterwards what to do next. And, as I always say: Get a scan to determine where exactly you have mets and how many!
My medonc Dr. Sartor who replaced Snuffy after he retired gave the following options when I pressed him during my last visit for next options if and when my triple ADT of Lupron/Xtandi/Avodart should fail - Jevtana, Testosterone therapy to reinvigorate Xtandi, possibly Provenge. He also told me that things are happening so fast that in 6 months when I was to see him again (I saw him in the spring) that we could have some new treatments to use so it didn't make sense to discuss in detail at the time.
I'm currently still on Snuffy's regimen of triple ADT, Metformin, estradiol patches along with several supplements - I've had chemo per CHAARTED and radiation to my prostate and several nodes back in 2014/2015. I've done Leukine for about a year back when I was seeing Snuffy. I've been watching the info on Lutetium which seems to be pretty promising and currently available in Germany.
I was DX in 2014 with Stage 4, Gleason 9 PCa. Saw Snuffy for a couple years, he was the best. So far PCa continues undetectable and scans clear but always thinking ahead for when that may change.
Hang in there! Sounds like you might be a candidate for Lu-177. There are now trials in Houston,Omaha, and UCLA. Lu-177 treatment has been utilized in Germany for a number of years with very good success.

After one Guardant360 liquid biopsy, it was revealed that my cancer had an ATM defect, so Dr. Charles "Snuffy" Myers could prescribe Lynparza, a PARP inhibitor. I got about 9 months from it.
Had Docetaxel/Carboplatin chemo for 6 cycles, ending in January 2018. My Med Onc and I decided to rechallenge with Xtandi, which had failed after a 12 month run in 2015-16. With the the grace of God Xtandi has worked for the last 5 months to keep cancer and liver mets in check. May work for you?
