The part that jumped out at me was this

"Individuals who live in an area where this service is being delivered can discuss their circumstances with their GP or diabetes team at their next appointment to find out whether it is suitable for them."

Hopefully, there will be members here who can action an appointment with their GP

.I was offered the 12 week NHS weight loss programme a while back, but the app was so clunky, with limited access to most of it, and no access to the videos that were part of the course. I gained a stone on the programme and there was no after care.

I will wait a year or so to see if my GP practice takes the new programme up as they are very slow at getting anything done

Of course, I would never advocate waiting for your next appointment. If it is something that might help you, and it is available in your area, contact your GP now!

My point was that I don't have the option of seeing a GP as he has been unavailable for appointments for several years and I don't know who my Diabetes team might be. I wish any members here well, who CAN see their GP though, and I know that your post will be helpful to some here

I was in a similar position of not being able to get a GP appointment. I hadn’t realised that my practice had been bought by a GP who managed a number of surgeries remotely giving mainly phone call consultations only. I have recently changed to a different practice after personal recommendations from neighbours who were getting appointments to see a GP. Very happy that I’ve made the change.

I recently approached all the practices in my area to switch practices, only none would take me on because I am housebound and can't get to surgery without support

I wonder if Drs still go out on their rounds? I haven’t asked for a home visit since 1984!

I rarely see any of our GPs either but will generally get a call from one of them. Something like this can be dealt with by phone in a lot of cases. In my surgery, as in many, Type 2 Diabetes patients are cared for by a Nurse or Nurse practitioner. Perhaps contact your surgery and just ask to speak to someone about your diabetes.

Thanks for your reply. I tend to find out about health issues and any new diagnosis by accident while checking my health record on my NHS app as my GP doesn't keep me informed. My health record changes every time I look it up, so even the app is unreliable. I learn about Diabetes by looking things up on Diabetes related websites now as my GP practice is pretty useless.

Are you getting the mandated annual diabetes checks? If not, you need to report this to the Integrated Care Board for your area. You might also want to tell the ICB about your inability to change practices.

I don't have any of the annual Diabetes Checks as I would need to leave my home and travel to a centre 20+ miles away. This would involve paying a personal assistant for her hours and travel expenses. I don't get funding for this so I have to prioritise which things I pay her to do.

In your circumstances then your Integrated Care Board should ensure that you are seen at home. You need that level of care.

Thanks...I had a look at the Integrated Care Board site and it's too difficult to navigate. Long story. I no longer have the IT skills I used to have before my brain injury and need a support worker, but don't meet the criteria for help
