The dreaded Capability for Work questionnaire

Hi folks, I posted a while ago about this and now, I have posted my questionnaire. It took me 3 weeks to complete, due to the amount of extra information I needed to submit. Anyone requiring a home visit in relation to this, needs to know that the health advisory people, i.e Atos or Maximus, state on this form that if you request a home visit, they will ask you for a letter from your doctor or consultant, stating why you are unable to travel. Just a bit more stress for people unable to work due to illness. When I applied for PIP, and thats a different story lol, this statement doesn't appear on the form so Im unsure as to wether they have changed their criteria for home visits. Just giving you the heads up, so you're prepared. Im in the waiting game now, waiting their reply.

Dear Suzycat,

This is a Very difficult question, to answer, I'm afraid. My advice is 'get as much information, about yourself/your Condition, as you can'. Be prepared to be Rejected and have to Appeal, sorry but this seems to be the 'Way' at the moment. Do be honest but, at all times, imagine that it is your Worst Possible Day. DON'T say, anything like, 'I can do this/that' or, worse still 'I normally do.....for myself'.

Do, by all means, speak to your Hospital Doctors/Consultants and, do bring, your GP into the 'Loop' too. The more Paperwork, letters/documents, you can bring the better.

I have had 'A Good Number' of these Assessments, over the Twenty odd years, since I fell ill so I please do ask a 'More Specific' question, if you need to Suzycat.
