Hi

I’ve not been tested for some time now and have finally managed to get t3 tested as well from GP . I have a gist of my results but would like a more expert option if someone would be kind enough to interpret esp the thyroid as not sure where the t3 fits in.

I’m diagnosed Hashimoto for 10 years.

Currently on 75ml liquid levothyroxine which is magic stuff for me, feel so much better on this and absorbing better. Feel ok overall apart from lack of sleep some nights.

Edit - Tested early morning with no levo taken 26 hours before blood test

I can’t seem to add all the results so here manually

TSH- 1.55 min/L (0.3-4.2)

T3 - 4 pmol/L (3.1-6.8)

T4 serum free 13.8 pmol/L (11.9 - 21.6)

Serum ferritin 106ng/ml (13-150)

Serum folate 3.2 nglml (>3)

B12 - 334 ng/L (200 - 771)

TSH- 1.55 min/L (0.3-4.2)

FT3: 4 pmol/l (Range 3.1 - 6.8) 24.32%

FT4: 13.8 pmol/l (Range 11.9 - 21.6) 19.59%

You are very under-medicated.

Your TSH is rather high for someone on thyroid hormone replacement, and your Frees are too low. Most hypos feel best when their Frees are in the upper quartile of the range. Yours aren't even in the bottom quartile!

And this is reflected by your nutrient levels.

Your folate and B12 are much too low.

Your ferritin looks good but was the CRP tested at the same time?

CRP - C-reactive protein - is an inflammation marker. If you have high inflammation it can give you a false high reading of ferritin. And with your low Frees, I'm guessing that you probably have high inflammation. So it's important to always test ferritin and CRP at the same time.

Thankyou so much for your reply. Gosh really! i don’t know how i will get gp to up the thyroid meds and to what dosage as they are all showing normal on the results.

I did wonder if I was ok as I’ve been on 75 for long time

I don’t think they tested inflammation markers on this but they did creatin

Request “trial” increase in dose

Unless extremely petite 75mcg is very low dose

Even if we frequently start on only 50mcg, most people need to increase levothyroxine dose slowly upwards in 25mcg steps (retesting 6-8 weeks after each increase) until eventually on, or somewhere near full replacement dose (typically 1.6mcg levothyroxine per kilo of your weight per day)

Adults usually start with a dose between 50 micrograms and 100 micrograms taken once a day. This may be increased gradually over a few weeks to between 100 micrograms and 200 micrograms taken once a day.

Some people need a bit less than guidelines, some a bit more

If symptoms of hypothyroidism persist despite normalisation of TSH, the dose of levothyroxine can be titrated further to place the TSH in the lower part of the reference range or even slightly below (i.e., TSH: 0.1–2.0 mU/L), but avoiding TSH < 0.1 mU/L. May be worth trying Vencamil Levo tablets

Lactose and mannitol free

Seem more easily absorbed than other brands

Perhaps ask for 25mcg tablets as an extra alongside liquid levothyroxine

No, they are not showing 'normal', they are showing in-range. Not the same thing.. There is nothing 'normal' about an FT4 only 20% through the range. 'Normal' (euthyroid) would be about 50% through the range.

Creatinine is a test for kidney function, not inflammation, so it doesn't look as if they did test it.

What B vitamins have you been taking

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement

A week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need 2 per day and/or may need separate methyl folate couple times a week

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 3-5 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12 if last test result serum B12 was below 500 or active B12 (private test) under 70

I came off everything I was taking medicine and vit wise two years ago except the levothroxine as I was getting so ill and not any better.

I needed a reset.

Sorry vit d is attached here. I’ve tried to stay natural . I usually use Spertie - admit not for a while I must start again and get in the sunshine. I can see by by results it’s not great

So should I be looking for upping thyroid to 100 ? I have no idea if gp will do that as all showing in range to them … hope I dont have a fight on my hands

I don’t take any b vitamins or anything fortified with them for prob last 6 months as I wanted to be free of any before taking the b12 test. I will look at your suggestions Thankyou

My folate was very low like yours & I felt awful with it that low...I take a folate supplement & it's now about 20 & feel much better with it up around there...all the best with it all.
