hi all. Recently increased from 75mcg to 100mcg, 5 weeks ago. Today I received the results. Please see attached photo. I’m concerned as to why the t4 is high but the rest in range?? Someone please reassure me! Health anxiety is so bad - please no horror stories! X

What were your results on 75 mcg?

Do you have Hashi's?

5 weeks was a bit early to test.

Results on 75 were

2.52

4.9

15 (same ranges)

Also just to add, I took my t4 (levo) 12 hours before the blood test x

ok, so that won't have helped. But that looks like Hashi's to me. Have you had your antibodies tested?

OK, so looks like you're having a Hashi's 'hyper' swing. It's only temporary so the best thing to do is to stop your levo until you feel hypo again. But, do it yourself, don't involve your doctor. Because if a doctor reduces a dose it's the devil's own job to get him to put it up again!

Do the t3 and tsh both look ok? Or could I add some t3 in (small amount) to see if it helps symptoms x

The TSH is a little on the high-side. Most people feel better with it under 1.

FT3: 5 pmol/l (Range 3.1 - 6.8) 51.35%

So, yes, there's plenty of room to try T3 - if you have easy access to it. If not, retest in a few weeks and see what your FT4 level really is. Could be that you just need an increase in levo.

It’s been 5 weeks and 4 days since the increase. So perhaps I’ll retest in 1 week and then consider t3? X

Sorry for all the questions, it’s just a really difficult time trying to look after a 4 and 2 year old especially at Christmas time with so much to do!! Thanks for all your help I really do appreciate it x

You're very welcome. :) Christmas can be a pain, can't it, when you're not feeling well. I've cancelled it this year but I don't have young kids, so it's easy to do. I just can't be bothered with all the fuss this year. :(

Ft4 high because last dose levothyroxine was too soon before test …..should be 24 hours

You also tested too soon

Retest in another 6 weeks and test correctly

Ft3 well within range

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Presumably you are taking levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

I think as you have tested 12h after taking the T4, this could have skewed the results. T4 is a storage hormone and has a half life of 6-7 days, so it gets cleared out of your system very slowly. Peak plasma concentrations are reached between 3-4h after ingestion and then slowly fall. So if you took a test 12h after levothyroxine intake instead of the 24h, more levothyroxine would have been present in your blood and therefore the T4 has gone over range.

I would do a repeat test in about 2 weeks time and leave 24h between the last levothyroxine intake, so the results are better comparable to the one you have taken before.
