finally today had my results from my endocrinologist appointment and the blood tests.

TSH 1 (0.27 - 4.2)

Ft4 17.2. (12.0 -22.)

Ft3 3.8. (3.1 - 6.8)

Tested negative for coeliac

I had a Synacthen test last week basal cortisol 265 rising to 513 this was because my cortisol levels were low..

She considers the above tests to be normal..

I reduced my Levothyroxine in September as my results showed slightly over medicated I was getting headaches and very hot..

TSH 0.234 ( 0.27-4.2)

T3 4.1 (3.1-6.8)

FT4 20.1 (12-22)

She has sent me a questionnaire regarding quality of life to consider a trial of T3 but I have to increase Levothyroxine before..

I’m at my wits end it’s taken me years to get this and I’m devasted.. as many poeple say it eroded any life I have..

I have severe leg pain almost all the time..both 2& 3rd toe nails yellow and thick like fungus

I take good supplements and my diet is good..

any advice would be appreciated

Free T3 (fT3) 3.8 pmol/L (3.1 - 6.8) 18.9%

How much levothyroxine are you currently taking

Current results show you are on inadequate dose levothyroxine so yes you need to increase dose levothyroxine first before trailing T3

Which brand of levothyroxine are you taking

Do you always get same brand

what are your most recent vitamin D, folate, ferritin and B12 results and ranges

is your hypothyroidism autoimmune

I’m taking Eltroxin 100/125 every other night..only started on this brand 6 weeks ago as I was getting different ones every 2 months.. 2nd I started getting a dry month and burning at the tip of my tongue

Yes I have Hashimotos..

The endo has said she want me to optimise my Levothyroxine and I. The next sentence said if we increase your Levothyroxine my TSH may become suppressed..

I was 1st diagnosed in 1991 stopped meds after 6 months as I felt worse..

Started again in 2003 never felt any better.. it’s taken all this time to see an endocrinologist.. so I’m desperate.. I’ve just requested what were my low cortisol levels..

I paid for medichecks last summer for levels and have been supplementing since..

I take Viridian B12 B complex it has 200ug of biotin I would have taken it 24 hours before my test would that amount make much difference?clearly it was an oversight by me..

I take my Levothyroxine during the night to optimise absorption..

So next test stop B complex 5-7 days before test

Instead that week take daily folate and separate B12

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

Last ferritin test last September 33 ( 10-300)

Ferritin virtually deficient

So it’s essential to work on improving and maintaining all four vitamins to optimal levels otherwise you won’t be able to tolerate adding T3

Serum ferritin level is the biochemical test, which most reliably correlates with relative total body iron stores. In all people, a serum ferritin level of less than 30 micrograms/L confirms the diagnosis of iron deficiency.

Never supplement iron without doing full iron panel test for anaemia first and retest 3-4 times a year if self supplementing.

It’s possible to have low ferritin but high iron

Test early morning, only water to drink between waking and test. Avoid high iron rich dinner night before test

Stop iron supplements 5-7 days before testing

Look at increasing iron rich foods in diet

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Thank you for your incredible patience while you have been awaiting the outcome of our ferritin reference range review. We conducted this with Inuvi lab, which has now changed the reference ranges to the following:

Females 18 ≤ age < 40. 30 to 180

Females 40 ≤ age < 50. 30 to 207

Females 50 ≤ age < 60. 30 to 264l

Females Age ≥ 60. 30 to 332

Males 18 ≤ age < 40 30 to 442

Males Age ≥ 40 30 to 518

The lower limits of 30 are by the NICE threshold of <30 for iron deficiency. Our review of Medichecks data has determined the upper limits. This retrospective study used a large dataset of blood test results from 25,425 healthy participants aged 18 to 97 over seven years. This is the most extensive study on ferritin reference ranges, and we hope to achieve journal publication so that these ranges can be applied more widely.
