hello. I have been on the forum before and feel like I have gone in a circle.

Current results

FT4 17 9range 12-22)

TSH 0.78 (0.27-4.2)

T3 3.5 ( 4-6.8)

Cholesterol still very high, they have wanted me on statins for years

these results are after i I went off all t3 and went on 75 of t4 for some months. Even I can see i am not converting at all which was why I tried the t3. Since t4 only, other clues include a much lower HR again , generally 45-48 in the night again suggesting no thyroid getting into the cell.

In the past, when I was advised to look here, I didnt feel bad but suspect i had been running on adrenaline for a very long time. Cortisol also skewiff., low early when it should be high and higher than it should be at night. , When I tried t4 before my conversion to t3 was also low, hence trying T3, and back then I had other symptoms eg cold, poor circulation, little eyebrow hair. Something else I assume is thyroid related is how high my cholesterol is.

The NHS endo had insisted I come off T3 and really wants me on statins. I wanted to see if by getting thryoid up my it would take up the very high cholesterol as i gather that is the process..

I feel I am back to square 1. Any thoughts greatly appreciated.

I assume I need to get all my vitamins and ferritin tested (ferritin was low last time). I assume taking t4 is a barely worth continuing. i didnt notice much difference except feeling low, in fact depressed, as I slowly re-started it and built up to 75.

I wonder if it is linked to adrenals/cortisol system too?

I do not feel as bad as many seem to on here, as I can exercise (e g. gym/yoga) most days and feel reasonably strong for a 66 year old.

Was test done as recommended

Testing early morning, ideally just before 9am, only water between waking and test and last dose levothyroxine 24 hours before test

75mcg levothyroxine is only one step up from starter dose and highly unlikely enough, especially for a bloke

Approx how much do you weigh in kilo

Guidelines on dose levothyroxine is approximately 1.6mcg per kilo of your weight per day

Obviously Ft3 is below range

FT4: 17.9 pmol/l (Range 12 - 22)

Ft4 only 59.00% through range

So get 25mcg dose increase in levothyroxine

Retesting in 6-8 weeks

Which brand levothyroxine are you currently taking

Teva is only brand that makes 75mcg tablets

Teva upsets many people

Meanwhile test vitamin D, folate, ferritin and B12

What vitamin supplements are you currently taking

When were vitamin levels last tested

Have you recently retested GFR ….likely still low with very low Ft3

very low ferritin

Have you done full iron panel test for anaemia

Test early morning, fasting, don’t eat iron rich dinner night before testc

Never supplement iron without doing full iron panel test for anaemia first and retest 3-4 times a year if self supplementing. It’s possible to have low ferritin but high iron

Have you tested BOTH TPO and TG antibodies if not get both tested privately

NHS won’t test TG antibodies as your TPO antibodies are negative

Are you on any other medications apart from Levothyroxine

all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Have you been tested for coeliac disease

Or are you already on strictly gluten free diet

yes test was all as recommended I am pretty sure

My weight is aprx 72 kg . I came off a low carb diet as I had hoped carbs would help thyroid. A stone + later it hasnt! According to your calculation i should be on about 116? i have enough to up my dose. I know its a starter dose but if my conversion is so bad is it worth upping it?

Re brands i think i have been given various brands, mainly teva and wockhardt? None upset me (or do much good I fear!).

I do take some supplements eg occasional b3, d sometimes, magnesium, at night melatonin sometimes, ecklona cava, reishi,, nac, vit k

Looks like I need some tests done, thanks for helpful information

Probably iron as well and private thryoid ones plus vits

(also i have never been tested for gluten, I dont eat much but it has never affected me)

only increase levothyroxine by 25mcg …..then retest in 6-8 weeks

Meanwhile perhaps test vitamin D now and full iron panel

You don’t need any obvious gut symptoms, can still find gluten free astonishing

Get coeliac test first though before trialing

Uffe has videos on YT. Malcolm Kendrick's book The Great Cholesterol Con is a good read. Relatively old now, but I don't think anything has changed to his way of thinking then. We've just had more of the same.

Statins block the HGA-COA pathway, an important enzymes CO Q10 is blocked. COQ10 is essential for the heart and life; look this up in images if interested.
