Help with ok latest blood test

Hi,My most recent blood results are:

Free T3. 5.9 pmol. Range. 3.1 - 6.8

Serum free t4. 23.5 pmol. ". 10.0. - 22.0

Thyroid stim. 0.02mlu/L. ". 0.30. - 4.50

Hormone

Serum urate level. 360umol/L. ". 200. - 430

I have the rs 225014 cc gene on dio 2 test. I am taking one grain of ndt but not any additional T3.

Could anyone help with making sense of the results. For example should I ditch the ndt and try out T3 only?

I did feel better overall on the ndt than before I began taking it in January 2023.

It’s unusual to see someone on NDT with an above range FT4. Did you ever get your NHS blood results from before treatment?

Have you also tested your key vitamins now and what were the results?

When was your last dose of NDT taken before this test?

Have your thyroid antibodies been tested?

Hi Jaydee1507,No I do not have any NHS results in the year prior to starting the ndt. I was due to have my blood taken on a set date in mid may but it was cancelled at short notice so not taking the ndt in the previous 24 hours was pointless. Then I suddenly got a request to get the test done with very little notice but because of the limited notice I had taken my ndt some 20 hours earlier.

I don't think my antibodies have been tested - if they were I do not have any result.

I did ask if my vitamins could be done but they weren't.

Is the above range ft4 significant?

Someone with no thyroid output on a low dose of NDT would usually have a very low in range FT4 as NDT as only a small amount of T4 in it. If you're not taking Levo as well then where is all that T4 coming from?

The usual recommendation for blood testing with anything containing T3 would be to take the last dose 8-12 hours before the blood draw otherwise the result will show a fasle low. The half life of T3 being quite short. So your result of FT3 is at 75.7% of its range and would be higher had you tested at 8-12 hours.

You really need to get the vitamin testing done to see where thats at. Also thyroid antibodies. It would also be good to see your results prior to taking any thyroid hormone.

Hi, I checked with surgery, vitamins not done. I wonder if the potential conversion issue is the main factor for me. That may be solved by T3 alone but the endo who advised my GP set no relevance to the genetic test saying it was just a new area of research that was still evolving. Regards

Do you have a diagnosis of hypothyroidism though? The DIO2 gene usually only comes in to play with someone needing replacement thyroid hormone who cannot properly convert Levothyroxine to T3. If you still have a functioning thyroid then I'm unsure where the DIO2 gene fits in.

Hi, going back a number of years I tested as under active. Then back to normal range so no treatment. I had the Dio2 test after years of feeling cold, tired, lethargic etc etc and being told that I was in normal range. Long family history of under active thyroid too. My sister had the same problems but is now being treated after developing nodules on thyroid. I do not know anything about the numbers for thyroid ranges which is why I was hoping members on the forum could give me some good practical based advice which I am grateful for. Maybe an issue is just down to conversion. Regards

Do you have any record of what your thyroid hormones looked like before NDT? As others have mentioned, 1 grain is not a large dose so it's surprising to see over the range ft4.

Hi, I don't have that information. Only ones I could track down were from 2018 and they were listed in normal range. Regards

Suggest you get full thyroid and vitamin testing privately if GP won’t test

Hi, Thank you. It seems this process of trying to resolve my difficulties is not running smoothly for me at all. I'm going to have a rethink about ndt, sort out vitamins etc and look closely at the antibodies. Regards

Your Levels don't look too bad the FT4 is high for NDT. I have no thyroid and had been taking 2.25 NDT for years. To feel perfect my level FT3 was always about 6.20 but like yourself my FT4 was always high but not over range. Do you have a thyroid.
