Blood test results help please!

Hi everyone!

Thank you for all your advice last time, I went back to my gp with a list of what I wanted to be tested for and to my surprise she said yes, except for doing a test for the T3!

Here are my latest results if you would kindly be able to advise me please?

I've noticed a massive fluctuation in my TSH results (compared to last time).

I am still on 200mcg Levothyroxine. I haven't heard back from my gp yet following my results and what they could mean.

I followed your advice of no water or food before the test and no medication 24hrs before the test either, I also wanted to book in as early as I could however there were no morning slots so had to go for the 12pm one.

Latest blood test results (20th Sep):

Serum thyroid peroxidase antibody concentration 224 IU/mL; Thyroid (TPO) Ab Ranges

Negative < 25 IU/mL

Equivocal 25 - 35 IU/mL

Positive > 35 IU/mL

Serum free T4 level 17.3 pmol/L [11.9 - 21.6]

Serum TSH level 0.13 mIU/L [0.27 - 4.2]; Below low reference limit

Serum ferritin level 157 ng/mL [30.0 - 400.0]

Serum free triiodothyronine level 4.8 pmol/L [3.1 - 6.8]

B12/Folate Haematinic assays

If anaemic and MCV raised repeat in one week.

If still low suggest therapeutic trial of folate.

Serum vitamin B12 level 355 ng/L [197.0 - 771.0]

Serum folate level 2.8 ng/mL [3.0 - 20.0]; Below low reference limit

Previous blood test results (25th April):

Serum free T4 level 16.8 pmol/L [11.4 - 21.4]

Serum TSH level 5.68 mIU/L [0.27 - 4.2]; Above high reference limit

What’s GP doing about these?

Folate is deficient

B12 is too low

No vitamin D result?

Ferritin is ok….but as bloke you might expect it higher

High thyroid antibodies confirms autoimmune thyroid disease…..aka Hashimoto’s

Presumably you knew this?

Have you had coeliac blood test done

Are you on, or tried strictly gluten free diet

I've heard nothing from my GP, considering these results were published 8 days ago on my NHS App.

My GP was going to do a basic blood test originally, it wasn't until I rang up and asked that I wanted the extra tests that they did it. (It would have been a repeat of what I had in June).

I did not know I have Hashimotos, like I said, I haven't heard from them. They will only give me a telephone appointment and refuse to see me face to face which can be frustrating (I'm in an area that is bad for phone signal).

I haven't been tested for Coeliac either, my IBS has flared up alot recently so I have been speaking with a dietician (privately) who has placed me on a FODMAP diet, which is helping and includes being gluten free, previous to that I was on a low carb diet anyway which I had been doing for around 9 months so my intake for gluten then was low. Now I am completely gluten and dairy free.

I've had problems with dairy in the past and told my doctor too, was just told to avoid dairy but they didn't do any tests for that either. Hence why I got a private dietician.

gluten intolerance is extremely common with Hashimoto’s….and dairy intolerance or dairy free diet often helps or is essential too

There’s no point having coeliac blood test. You would have to go back on high gluten diet for minimum 6-8 weeks. Only 5% of Hashimoto’s patients are coeliac, but approx 80% find strictly gluten free diet helps or is essential. You know you’re gluten intolerant

Which brand of levothyroxine are you currently taking. If lactose intolerant you really need to be on lactose free levothyroxine

Aristo or Teva are the most easily available lactose free levothyroxine

Glenmark or Aristo (100mcg only) are lactose free and mannitol free. May be difficult to track down Glenmark, not been available very long 

Many patients do NOT get on well with Teva brand of Levothyroxine.

Teva contains mannitol as a filler, which seems to be possible cause of problems.

But for some people (usually if lactose intolerant, Teva is by far the best option)

suggest you get vitamin D tested now

are you currently taking any vitamin D supplements?

Test twice yearly via NHS private testing service when supplementing 

vitamindtest.org.uk

Aiming for vitamin D at least around 80nmol minimum

make an appointment with GP to discuss deficient folate

They should prescribe folic acid

When folic acid prescription is finished

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) 

This can help keep all B vitamins in balance and will help improve B12 levels too

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) 

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate methyl folate supplement and separate B12

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months.

once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

hello shaun. Well done to your GP for being able to get all those tests done. The lab my GP uses would have vetoed all requests but the TSH on the thyroid ones! Lol...

You need to get your B12 over 500 as a minimum, preferably upper part of range.

Shame your GP didn't do coeliacs test before you excluded gluten. However testing for coeliacs does NOT test for gluten intolerance. Gluten intolerance can only be tested at a specialist Ataxia clinic.....only two in England. Am glad you are feeling better on Fodmaps regime.

Vitamin D test your GP can do. Have a look at the vitamin D Council they have some really great informatiom on vit D and recommended range. Symptoms of vit D are fatigue, non restorative sleep ie wake up feeling tired and aches and pains
