Thyroid lumps, high antibodies but ok thyroid tests.

Hello,

I found a lump on my neck and the scan showed multiple colloid cysts the largest is 2x3cm (still waiting for the report) the Gp blood tests are below which show thyroid is functioning ok but ferritin and folate are on the low side.

Serum folate - (SL512) - 6.7 ug/L

2.90 - 50.00ug/L

Serum ferritin - (SL512) - 39ug/L

30.00 - 200.00ug/L

Serum TSH level

0.87mU/L 0.30 - 4.20mU/L

Serum free T4 level

14 pmol/L 12.00 - 22.00pmol/L

I also had a blue horizon thyroid check which has shown high thyroid antibodies, but my thyroid is functioning ok.

The Gp will say all is fine but do I need to leave it for now? Push to see and endocrinologist? Retest in a few months?

Important to also test B12 and vitamin D

Ft4 is on lower end of range.

Hypothyroidism tends to cause lower stomach acid and this often results in low vitamin levels

Low ferritin

Look at increasing iron rich foods in diet

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Don’t start any folate supplements or vitamin B complex until got B12 tested

I was also put on iron tablets by the Gp in October so took those for two months then a lower dose of iron ever since.

Vitamin D deficiency is frequent in Hashimoto's thyroiditis and treatment of patients with this condition with Vitamin D may slow down the course of development of hypothyroidism and also decrease cardiovascular risks in these patients. Vitamin D measurement and replacement may be critical in these patients.

The thyroid hormone status would play a role in the maintenance of vitamin D sufficiency, and its immunomodulatory role would influence the presence of autoimmune thyroid disease. The positive correlation between free T4 and vitamin D concentrations suggests that adequate levothyroxine replacement in HT would be an essential factor in maintaining vitamin D at sufficient levels.

So B12 slightly low, low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)

Or Thorne Basic B is another option that contain folate, but is large capsule

If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

With B12 result below 500, recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months, then once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.
