New here: Low TSH, low Free t4, and high Free t3?

In New Zealand,

latest blood results for hypothyroid are:

Free T4: 12.7 pmol/L (12-22)

TSH: 0.96 mIU/L (0.27-4.20)

Free T3: 7.5 pmol/L (3.9-6.8) H

am on eltroxin 1 daily 50mcg, and Liothyronine 10 mcg in am, 10 in mid afternoon.

Still gaining lots of weight (will be well set if a famine sets in), and thinning hair. Can not loose wait even on OMAD. I can't make sense of these bloods.

If anyone can give me a pointer, that will be great.

Can't see how to edit this. I had a cortisol test done a couple of years ago and are awaiting the results of another one. It showed cortisol that was sky high. I doubt that a lot has changed...

How long did you leave between your last doses of levo and T3, and the blood draw? TSH and FT4 are bound to be low when taking 20 mcg T3. But, if you didn't leave at least 8 hours between your last dose of T3 and the blood draw, that could explain your high FT3. :)

Day before test ....best to split 2nd dose into 2 x 5mcg

Taking 5mcg mid afternoon and last 5mcg 8-12 hours before blood test

Do you have Hashimoto’s?

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially if you have autoimmune thyroid disease (Hashimoto's) diagnosed by raised Thyroid antibodies

When were vitamin levels last tested

What vitamin supplements are you currently taking

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

If/when also on T3, make sure to take last third or quarter of daily dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test

Is this how you do your tests?

Many of us on levothyroxine plus T3 find we need GOOD Ft4 and Ft3

You might need to increase levothyroxine dose a little to bring Ft4 levels up

Thank you for your response. There was 2 hrs between taking the t3 and the test which explains why the t3 reading is high. Am going to a naturopath now who is questioning my vitamin levels.

We will see. Almost feel like it is like divining for water :)

You need to TEST vitamins

Extremely common to have low vitamin levels as result of low stomach acid from being hypothyroid....especially if cause of hypothyroidism is autoimmune thyroid disease

What vitamin supplements are you currently taking

We frequently need to supplement to maintain optimal vitamin levels
