Help! Don't have the monitorymyhealth compliment slip to submit with my sample...

I did the blood sample for monitormyhealth TSH, T4 and T3 early this morning. I found I did not have the compliment slip with the tear-off part to submit with it. I have emailed them for advice but their auto-reply states they may not respond for 24-48hrs and there is no phone number. Please does anyone have a sample kit who could tell me what type of information is required - and I could maybe submit it using my own references on a different piece of paper? If it is a bar code or QR code then I'm stuck, I think, but if just numbers/words, I may be able to do it. I have done this test before but I can't remember what the slip looked like. Thank you very much for your help.

Rather than delete my post, I thought I would let you know that I have just had an email from MonitorMy Health (20 minutes after my emailing them) and I can return the sample with my name, and the date and time of the sample. Phew! Thank you everyone.

I’ve just sent my test off and can confirm that the only info needed on the slip was the date of the blood draw. But I managed to post the info leaflet back with the test! Was there any info on that, please, about how and when to access the results, or anything else I need to know? Thanks for having a look at it for me!

Hi Bearo

Thanks for your reply. I only have the Blood Collection Instructions. On the back it has Returning Your Sample and at the bottom of that section it says: "You will be contacted by us in approximately two days. Your results will be available to view on your password-protected online dashboard. Please contact us through monitormyhealth.org.uk if you have any queries."

You will get an email from them telling you that your results are ready to view and then you can log on and can see them.

If you aren't sure, please come back to me.

Sometimes it takes a bit longer because of the post.

Bearo, You will receive an email from MMH as soon as your results are ready. All you need to do then is log in to your account to see the info. They are usually very quick so you might get the email tomorrow or the day after if you posted your test today.
