A bit fed up and miserable at the mo as still feel like crap. Started on Levothyroxine in January and still trying to get the right dose. I was on 100mcg levo but reduced this to 75 and added 10mcg of T3 a month ago. Endo suggested adding in T3 a month ago which has had an effect but I’m still under medicating I think. Have a phone appointment with endo this week to review.
What would you do with results below? 49% and 2% through range. I’m thinking back to 100mcg levo and some more T3. I take both in the morning and this test was 24 hours after last dose (which was what doctor instructed) and fasted. B12 and folate is low. Vit d ok.

As usual the Doc reduced your Levo when it didn’t need reducing. I’d get back on the 100mcg and then maybe another 25 after that, see what the blood test says. And obviously work on this Vits/minerals being optimal. I know people say do one thing at a time but Ain’t nobody got time for that, work on these all. Then when they’re all optimal look at your T3. HTH 🤗
If this result was 24 hours after your last dose of T3 then you have a false low reading. For T3 the last dose is 8 - 12 hours before.
Given the false low I would not alter your dosage of T3. I would increase your levo by 25mcg. then bloods in 8 weeks. The additional levo will increase your FT3 as well depending on how well you convert.
Some folks need their FT4 well over halfway through range even on combo. You could try a couple of increases of levo over time in my opinion. Bring that FT4 level up and then see where that leaves your FT3.
Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .
Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).
This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)
If/when also on T3 or NDT make sure to take last half or third of daily dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test
So you needed to split 10mcg into 2 x 5mcg and take last 5mcg around 9pm evening before test
Many people find it better to split T3 into 2 doses every day
What are your actual results and ranges for vitamin D, folate, ferritin and B12
What vitamin supplements are you currently taking?
I take b12 complex, selenium and zinc (off and on), methyfolate, vit d (20K ui twice per week at the mo, will drop down in a few weeks/months), magnesium. B12 is low because I haven't eaten much meat - usually eat when out rather than home, and obvs haven't been out much! Also think I need to take two igennus or swap back to Jarrow which I think was better for me. Was also going to switch a zinc tablet which includes a bit of copper to balance out - have found one on cytoplan.
Ferritin has been consistently high - I had a full iron panel and based on those results both endo (who is generally a pretty good one) and GP said I didn't need to be genetically tested for haemochromatosis. It was endo that had pushed GP to check this out in first place.
I've been taking levothyroxine for about 15 years now. My doctor gets my blood tested every few years now. I was on 100mgc for the first few months, then up to 150mgc. Then I was put on a reduced dose of 125 and then back to 150. I've been on that for about 4 years now,
