what does this entail when T4 is high..
That depends.
Are you diagnosed hypothyroid and on thyroid hormone replacement? If so it could mean that you are overmedicated.
What are the reference ranges so we can see if your FT4 is over range.
Was FT3 tested? That's the important test and can tell us if you are converting T4 to T3 efficiently and it also can tell us if you are overmedicated.
If you are not diagnosed hypothyroid then a high FT4 can sometimes point to autoimmune thyroid disease, either Graves Disease (overactive) but your FT4 would probably be a lot higher and your TSH would be supressed if it was. Or it could be that you have Hashimoto's (immune system attacks the thyroid and eventually destroys it - hypothyroidism) but that you are going through a slightly hyper phase, with Hashi's test results/symptoms fluctuate with the Hashi's activity.
So maybe if you could tell us a bit more we can be more specific with a reply
Hypothryroid yes tken 100 mcg levothyroxine have hashimotos , I had my throid checked as been hyper lately not sleeping etc all doc said was will take again 2 months time. I did think as I have lost over a stone in weight due to illness that maybe need the dose reduced. the doctors here do not do all the tests so these is the nly ones they did as I had asked for thyroid check.
Have you got the reference ranges, these vary from lab to lab and ranges we see here are:
7-17
9-19
11-23
12-22
and others.
So we don't know if you are high in range, slightly over range or a lot over range.
and others
And before changing your dose of Levo, you should get FT3 tested at the same time as the others. If your GP doesn't test it, and you don't feel a reduction is necessary, then refuse until it is tested to prove overmedication, or get a thyroid panel done privately at one of our recommended labs who provide home fingerprick tests or venous blood draw at extra cost.
If you have lost weight due to other illness, and not thyroid, then that's not a reason to reduce your dose of Levo.
Also, there is the normal fluctuations that Hashi's causes, which are temporary, and sometimes a temporary alternation of dose is necessary - just the dose not the prescription because eventually the original dose will probably be needed again.
Are you using t3 with that high t4 count??
Have you had vitamin levels tested?
Do you have Hashimoto's?
For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12
Low vitamin levels are extremely common, especially if you have autoimmune thyroid disease (Hashimoto's) diagnosed by raised Thyroid antibodies
Ask GP to test vitamin levels (and thyroid antibodies if not been done)
You may need to get full Thyroid testing privately as NHS refuses to test TG antibodies if TPO antibodies are negative
Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .
Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).
This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)
Is this how you do your tests?
Private tests are available. Thousands on here forced to do this as NHS often refuses to test FT3 or antibodies

For thyroid including antibodies and vitamins
Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have special offers, Medichecks usually have offers on Thursdays, Blue Horizon its more random
Cheapest option for just TSH, FT4 and FT3 £29 (via NHS private service )

If you have been unwell, then vitamin levels may be low
Low vitamins impair conversion of FT4 to FT3
Yes I have had all tested previous I take vit D now and for life, TPO positive yes, doctor will not do the T3 . my T3 was ok last year but t4 was 23 so little higher now . only asked for a test as feeling hyper and lost weight
