Lates blood results for Oct’19

What is happening? Why is my TSH going up....last results were

Tsh-0.22, t4-21, t3-3.9, vit D-54, folate-15.2. B12-944, ferritin-91.

Ranges are in latest results. Folate is lower, B12 lower but I’m having another injection soon, t4 lower and t3 up slightly. I see Endo on Wednesday so need to be able to explain results. Many thanks.

Ps. Does it mean I need and increase in Levo or definitely t3.

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)

Is this how you do your tests?

FT4 is 62% through range

FT3 only 13% through range

Looks likely you need T3 added

Though you have room for dose increase in Levothyroxine first perhaps

You jogged my memory, I now take t4 in the evening between 9-10pm and blood test was 8.30am so not 24hrs, Derr trust me.x

So next time .......

If testing Monday morning....delay Saturday evening dose until Sunday morning.....

Then delay Sunday evening dose until after blood test Monday morning

Take Monday evening dose as per normal

So your FT4 is really lower than that result shows, so you have room for dose increase in Levothyroxine

How much Levothyroxine are you currently taking?

Levo 100mcg at the moment, felt better than I did a few weeks ago. Some days I’m fine and others so dizzy but that has calmed down a bit now. I refused to go down to 75mcg when saw my GP as she’s going by tsh result of 0:22. I wrote a letter saying I wasn’t going to go back to 75mcg because it would make me feel worse. P e other GP rang because of results and said do 75mcg one day a week which showed me they don’t understand the treatment of thyroids.

Looking at previous posts see you had TT

Many people after TT need addition of small dose of T3

Email Dionne at Thyroid Uk for list of recommended thyroid specialist endocrinologists who will prescribe T3 (if your endocrinologist refuses)

Yes got that from Dionne but it’s trying to get an endo to prescribe when they are adamant it’s too expensive. Wednesday will be difficult if they are adamant but I see a different one at a different hospital in January who used to be on the list, last time I look the name wasn’t there but I’m hopeful. I’m taking lots of stuff from here on Wednesday so wish me luck.x

Your vitamin D is too low as well

GP will only prescribe to bring vitamin D up to 50nmol.

Aiming to improve by self supplementing to at least 80nmol and around 100nmol may be better .

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Retesting twice yearly via vitamindtest.org.uk

Vitamin D mouth spray by Better You is good as avoids poor gut function.

It's trial and error what dose each person needs.

B12 on the way down again, endo did say I was deficient in that. I had injection for first time 8 weeks ago and it brought it up well but it’s going down again and even though slightly Iv started to feel really tired again,

As well as having B12 injections are you taking a good quality daily vitamin B complex?

Supplementing a good quality daily vitamin B complex, one with folate in not folic acid may be beneficial. This can help maintain B12 levels between injections

B vitamins best taken in the morning after breakfast

Igennus Super B complex are nice small tablets. Often only need one tablet per day, not two. Certainly only start with one tablet per day after breakfast. Retesting levels in 6-8 weeks

Or Thorne Basic B or jarrow B-right are other options that contain folate, but both are large capsules

If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before any blood tests, as biotin can falsely affect test results

How much vitamin D are you currently taking?

The fact vitamin levels drop again suggests you need T3

Hi I take better you spray but maybe I should up the spray to 3. I was wondering as I have sjogrens maybe the dry mouth is affecting the sprays as parotid glands are not 100% working and swell at times could it be blocking the sprays absorption what do you think?

Are you supplementing any magnesium ?

I was taking glycinate as I have a bowel problem but it has stearate In it, maybe only a small amount but it triggered an upset stomach, but most meds have that like omeprazole but stopped taking that now and complex B ingennus does so had to stop that, I think thorns is ok so must order some. I think d spray was 3000.
