You may not be able to improve it. After 45 years on thyroid meds, with a suppressed TSH and perfect FT4/FT3 at 75% through range, my tongue is scalloped, has been for years.

Hi Susie great to hear from you .. i had that feeling you might say something like that... Its not bad i dont think but you can see it... One of them things we have to deal with i gess

My dentist mentioned it a couple of years ago. She identified that the pattern coincided with how my teeth were spaced out - some of my bottom teeth are fairly spaced out as some double teeth missing. She concluded that my tongue was a bit larger than normal hence the scalloped edge following the pattern of my teeth.

I don't have a scalloped tongue but was told by a dentist that your tongue adapts to the size and shape of your mouth. This is apparently why it's often difficult to get used to dentures because they take extra space; but if you persevere your tongue shrinks slightly to fit

It can take a long time for thyroid hormone to achieve its peak effect.

Have you noticed any improvement? There are other causes of swollen tongue (macroglossia) which might need to be considered.

Hi hellvella ..i thought it was a bit better but my partner doesnt think so... Ive been on 100mcg for about 9weeks now..

My tongue did become a bit scalloped - and it felt wrong for me.

From where I am now, I really don't remember how long it took for me to recover but I do know it was quite some time. If you are still under-dosed, then it is unlikely to resolve until you approach adequate dosing. From TSH alone, which is far from ideal, it does look as if you are getting there.

Perhaps some questions to ponder over?

Have you checked T3 and reverse T3? This gives you a baseline from which to work and lets you know if you have a peripheral conversion T4 to active T3 challenge.

Are you clenching or grinding your teeth? This could indicate underlying, undetected infection and exposure to toxins which the pathogens utilise. If so Dr Jay Davidson’s work and parasite protocol is worth investigating on Youtube. (Amrita Nutrition or Beaumedltd).

I no longer have a swollen scalloped tongue.

Hi there sorry for late reply... my t3 etc was in middle to 3/4 range as far as im aware.. i dont grind my teeth as far as i know

Another long shot might be a H Pylori infection which is thought to contribute to low stomach acid conditions. Leading to possibly low B12 (methyl or adeno - cobalamin replacement) and low folate B9. Also a hiatus hernia with ensuing heart palpitations as the Vagus nerve becomes irritated.

This sets those of us who are clinically hypothyroid i.e. not converting easily T4 to T3 or T4 going t reverse T3. at a disadvantage.

As I said before worth pondering over and reversing.

I’ve had a scalloped tongue for two decades now, diagnosed with Hashimotos 17 years ago. I also have a disgusting white coating on it. It is vile so I don’t stick my tongue out much. 🙄🤣

Doesn't a scalloped tongue suggest B12 deficiency?

MariLiz profile image
MariLiz in reply to Gingernut44

Yes you’re right, mine improved once I was given B12 injections! It can sometimes be enlarged and sore too from low B12.

My b12 was in range low end of b12 .. im now on b complex ..

I suppose it could do, I've not looked into that, but my B12 level is excellent, Active B12 is 162 (37.50-188)

Thanks Seaside Susie, it was just a suggestion but there are probably many reasons why some of these symptoms appear, it seems you have to be your own detective 😊

My tongue was scalloped before my TT 8 years ago now, though I was told at the time it was nothing to do with thyroid. My B12 levels were also low but are now optimal. Although my tongue has significantly decreased (I had to sleep with it hanging out of my mouth at one stage as it was so large!) it remains slightly scalloped. I guess as I have other lingering symptoms it may be to do with that. My daughter had a swollen tongue from low folate levels and it took 3 months of supplements for hers to start improving. I think my tongue was the last thing to start improving so perhaps it is a little to early to see a change yet. Hope it does soon.

I would suggest the problem maybe low levels of one of the B vitamins. Which B Complex are you taking and how much of it daily?

Spotydave profile image
Spotydave in reply to humanbean

I forgot to mention... Iron is also worth looking into for scalloped tongue. Not just ferritin, but serum iron and transferrin saturation too.

Spotydave profile image
Spotydave in reply to humanbean

Is that out of norm nhs test ???

humanbean profile image
humanbean in reply to Spotydave

The NHS has done a complete iron panel for me a long time ago (decades ago), but I wouldn't expect to get one these days due to lack of money, even when it is justified.

It is possible to use fingerprick private testing :

Note that you can't combine money off offers.

How did i know you was going to say that .. lol

Its def something to look at

Zinc deficiency may be linked to a serrated tongue. Taking zinc with vitamin C has helped zinc levels which can also be recognised by a groove in the tongue. Zinc and selenium help to activate your thyroid as you know helping in the conversion of T4 - T3. Macroglossia is also linked to another deficiency but can' remember what it is!
