Tsh climbing after hysterectomy

I had a total hysterectomy 2 years ago and have felt unwell everyday although I am on hrt! I have severe insomnia, loss of hair/eyebrows, feel absolutely exhausted, dizzy, heart palpitations, pruney fingertips.I’ve just found my blood test results and last year my Tsh was 1.38 miu/L. Today it is 3.45 my Free T3 is 3.74 and free thyroxine is 18.9

Just wondering if this is why I feel so off!!

Are you on Levothyroxine? Or not diagnosed at all?

For full Thyroid evaluation you need TSH, FT4, TT4, FT3 plus TPO and TG thyroid antibodies and also very important to test vitamin D, folate, ferritin and B12

Private tests are available. Thousands on here forced to do this as NHS often refuses to test FT3 or antibodies

Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have money off offers.

All thyroid tests should ideally be done as early as possible in morning and fasting. When on Levothyroxine, don't take in the 24 hours prior to test, delay and take straight after. This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, GP will be unaware)

If antibodies are high this is Hashimoto's, (also known by medics here in UK more commonly as autoimmune thyroid disease).

Are you on Levothyroxine? If so TSH should be around one, FT4 towards top of range and FT3 at least half way in range

Can you add the ranges on these results please

TSH looks slightly high and FT3 low. Need ranges to see for definite

You also need vitamin D, folate, ferritin and B12 tested

Tsh. 0.27- 4.00

Free Thyroxine. 11-22

Vit d, b12 ok

Not on meds xx

Your FT3 is low in range. But FT4 is good

TSH borderline.. we would say it's too high...but medics very unlikely to agree

Without raised antibodies, very unlikely for any medic to consider a thyroid issue

Getting vitamins tested and optimal is next step

Insomnia and low vitamin D are often linked

I’ve been taking vit d 1000 for quite some time now and my b12 is around 450. Hopefully seeing a endocrinologist very soon

what was your D reading. my thyroid doc recommend 5000mcg a day he said any less was just to stop us getting rickets. there is a few social media groups about Vit D its interesting reading.

Can’t remember but was told ok. It was my menopause consultant that told me to take vit d .

You need to retest twice yearly, certainly annually

Often 1000iu is not enough. People vary wildly in the amount they need. Thyroid often needs higher dose, especially with Hashimoto's, or gluten intolerance or other gut issues

Some need 1000iu, others 5000iu or even higher. But vitamin D is toxic in excess so can't just take high dose

Don't expect endocrinologist to be interested in vitamins, they are often completely disinterested

What was vitamin D result? Around 100nmol is what vitamin D council recommends

Also B12 is better above 500 (range usually 210-900 approx)

Have you had folate and ferritin tested? Important to test before starting any supplements for B12 or any B vitamins

I agree with humanbean, As you said you had a total hysterectomy. This tells me they also removed your ovaries. Doctors rarely seem to think it is important to check our progesterone, estrogen, testosterone levels after losing our internal sex organs. But checking to see that our hormone replacement therapy is the correct type and amounts is just as important as having the correct thyroid hormones. And yes, ladies also need testosterone and often gyn hormone replacement is a "one size fits all" prescription. Also all the hormones-even if made by different systems- are interconnected and can have a bearing on levels of other organs. Some gynecologists are not always as well-versed as they should about the best hormone replacement therapy after a hysterectomy. I had a total hysterectomy in 1999 and my hormones were checked by an endocrinologist not my GYN surgeon. All hormones are part of our endocrine system though some are made/excreted by other organs (thyroid/pancreas/adrenals/ovaries/pituitary. If it were me I would get a second opinion about my hormone replacement meds (and also my thyroid) from an Endocrinologist. Just my opinion. Take care irina

Hi Jemcam. Your post was interesting and I did some research online. I googled 'what is the connection between ovarian hormones and thyroid hormones.' Got several interesting websites and one I liked was <bewellnaturalmedicine.net> It refers to the connection between our ovaries, our adrenals, and our thyroid calling them 'The Holy Trinity'. A statement the site made was 'the relationship between these three glands is like an equilateral triangle. Might be worth a quick look. Gosh this is like reading about me! I had my hysterectomy in March (the gyny pain has improved) but still feel horrendous day to day. I did keep my ovaries but know my hormones are totally out of whack as only have about 5 days a month where I can manage to get up. My TSH was 1.7 last year, 1.8 in May and now it is 4.5 but Dr's have just dismissed it saying it's normal to fluctuate and wants to refer me to a gastro specialist!!!! I'm now getting full thyroid and hormone tests done privately as know something really isn't right for how poorly I feel and the fact it is affecting so many organs and following the same pattern. A friend of mine had the same thing after a full hyst and said once her estrogen was at the right level her TSH automatically came back down. It's definitely all linked! X
