I've been on T4 for 15 yrs (250mg). Results of new test:
TSH 0.961
FT4 23.3 (12-23)
FT3 2.92 (3.1-6.8)
TPO 9.64
TGA 17.6
Folate, B12, Ferritin, D all good. I feel ok but should I take T3 because of poor conversion? I seem to need more clothing than most people to keep warm. 2 years ago, when I started supplementing B12 and Folate, T4 (T3 not measured) shot up to 34 but I felt no hyper symptoms (doctor alarmed and reduced dose to 175mg).

You might have a general conversion problem as your TSH is not very low given your fT4 is high. This can be caused by selenium deficiency, so I would either get your selenium level checked or supplement. If this doesn't solve the problem then I would consider reducing your levothyroxine and adding in some liothyronine. It would be better to eliminate the possibility of selenium deficiency before taking T3 medication.

Taking B12 and folate would not make your FT4 shoot up to 34. Sounds more like Hashi's. Have you had your antibodies tested? If you do have Hashi's, that could account for your poor conversion. But, it's a good idea to take selenium to lower antibodies, as well as improve conversion.
