Help with Interpretation please

Hi All, can you help with your thoughts on my latest blood results through my GP.

TSH 0.04 mIU/L "Low" and T4 14.7 "within range".

No check for T3 or anti-bodies. My thyroid hasn't been right for years, particularly following the traumatic birth of my daughter 10 yrs ago, no doubt everything got turned upside down for a good 6/7 yrs of that time due to high stress 24/7. Been on 75mg Levo for 24yrs. I am 49. I just need someone to help get me back on track. I need to be as well as I can to look after both my daughter, and also my husband who has MS. GP referred me to Endo which has been rejected!

Hello Mandy2007 could you please add the laboratory ranges for your blood test results? We can't really comment without them as lab ranges do vary. Have your antibodies ever been tested in the 24 years you've been on levothyroxine?

Are you registered online to view your blood test results?

Hi apologies for my ignorance......what laboratory ranges do you need? Just for Thyroid? If so, TSH range (0.30-4.80) & T4 range (7.7-20.6). I hope that is what you meant?

I have never (from memory) ever had my anti-bodies checked by NHS. I did go private around 3.5 yrs ago and I would imagine they were checked then. I don't have the paperwork to hand right now, but I can look tomorrow to see if they were checked. Since 2014 I have always got a print off of my blood results. Thank you for any help. Mandy

Thank you, yes, those are the lab ranges we need. It helps if you post them next to the blood test results like this.

TSH 0.04 mIU/L (0.30-4.80)

T4 14.7 (7.7-20.6)

From this I can see that although your TSH is low your T4 is is well within range and so it looks like you are not overmedicated but it would help if you had an FT3 result and antibodies too to view the bigger picture. If you have antibodies your blood test results might fluctuate.

How do these results compare to your last results?

June 2015 - TSH 4.3 (0.30-4.80)

T4 9.0 (8.4-19.1)

Apr 2016 - TSH 0.60 (0.30-4.80)

T4 12.1 (8.4-19.1)

Mar 2017 - TSH 5.56 (0.30-4.80)"high"

T4 7.8 (7.7-20.6)

July 2017 - TSH 0.04 (0.30-4.80) "low"

T4 14.7 (7.7-20.6)

Mandy2007 you say you've been on 75mcg levo for the last 24 years. Your results make me think you have thyroid antibodies as they are fluctuating. You really need to find out if you've got thyroid antibodies, either get your doctor to look back at results or get a new blood test to find out. TPO thyroid antibodies and TgAb thyroglobulin antibodies.

Have you felt unwell intermittently and are you taking any other medication? Do you always take your thyroid medication on an empty stomach leaving an hour before eating and drinking (except for water)? Any other medication should be taken 4 hours after your thyroid medication.

Two things spring to mind

You say you had traumatic birth of your daughter. Did that include a lot of blood loss or very low blood pressure due to trauma?

If so, could you have Sheehan syndrome?

More common and typical of autoimmune thyroid disease (Hashimoto's) is that it's extremely common to have low vitamin D, folate, ferritin and/or B12. In range result is not the same as optimal. Always get actual results and ranges

Essential to test both TPO and TG antibodies too

If you can't get full thyroid and vitamin testing from GP

Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have money off offers.

All thyroid tests should be done as early as possible in morning and fasting and don't take Levo in the 24 hours prior to test, delay and take straight after.

The reason you need to know if it's Hashimoto's is that very likely hidden food intolerances can be causing issues, most common by far is gluten.

Changing to a strictly gluten free diet may help reduce symptoms. Very, very many of us here find it really helps and can slowly lower antibodies.

Your FT4 result is a little under what you are aiming at. It should be in the top third or even in the top quarter and it that is the. Saw then your FT3 could also be lower in its range if it is shown you have a conversion problem i.e. FT3 much lower than FT4 in their respective ranges so get the other thinks checked as suggested and then post you results and any comment and suggestions your GP makes. We often are low so have to supplement. So once we know that we can advise further.
