How much reduction in dose to lower TSH to please the specialist???

Last Sept I had extensive excision surgery and the removal of one ovary. I was given utrogestan capsules and evorel 50 patches. Orgovyx + Abiaterone?
I have very high risk prostate cancer (Gleason 9, negative PSMA for spread, MRI suggests possible spread to seminal vessel, PSA in 7s before treatment).
