Artificial Urinary Sphincter (AUS) for older guy
My husband is 77 & totally incontinent following RALP (2014),
IMRT
& ADT (2016), several cystoscopies, & most recently following serious blockages Foley catheter for 2 months (increasing diameter size for dilation). The Foley was removed Oct 7. We were devastated that he's totally incontinent.

Questions for the RO for my upcoming video conference regarding options for early salvage treatment (and ADT) after Radical Prostatectomy?
I know there are many options for salvage treatments I have been reading about on this forum besides
IMRT
and proton like SBRT, VMAT, Hypofractionated radiotherapy, HIFU, Cyberknife, real time MRI guided radiation therapy, Elekta, etc.

Had Radiation and adjuvant ADT for 16 months now what?
The doctor wants me to continue on Firmagon for 24 months after the
IMRT
as a high risk case. It seems to be the SOC for high risk cases. I have been very regular in walking and exercising including some light weights.

Artificial Urinary Sphincter (AMS 800) - A first person account.
So, at my insistence, we went forward with 8 weeks of
IMRT
at about 3 months post-RALP, rather than the 6 months recommended for continence improvement.

Erleada without ADT?
Had
IMRT
to prostate and pelvic lymph nodes in Dec 2015. PSA in August 2019 was 0.15 but dramatically rose in August 2020 to 2.42. Previously been through 3 short cycles of ADT between 2012 and 2016, with intolerable effects on my QofL.

Nervous about Axumin MRI results
RALP 4/14; ADT &
IMRT
2-3/2016 Low PSA but doubling every 6 months: 0.5; 0.8; 0.18 Plus severe lower back & cervical pain. Only after I pressed him, did the urologist order the test, which is tomorrow. Uro also dismissed my saying it is time to get a medical oncologist!

Why did they let me off the ADT hook so easily?
Well after a year on ADT as part of a clinical trial at a major center of excellence, I met online with the RO today for a follow up (I had 37 treatments of
IMRT
in February).

any thoughts about Davallia formosana for PCa?
I am 12 years post RP, and post 6 years infusion with samarium-153 with
IMRT
with no known metastasis but with PSA trending upward now at 0.5.

What's Next?
Ten years ago surgery, then
IMRT
, several years of Lupron/Bicalutamide, Provenge, Radium 233, Taxotere and currently Jevtana. I am Stage 4 with bone/lymph node mets. PSA rising after 4 infusions of Jevtana (180). Bone scan/CT update in two weeks. Lost 35#, but still vertical.

Just an update on this journey
Perhaps this is a mistake since I did have one bone met (SBRT at Mayo in 2018), and I had failed both RP and
IMRT
. I will have PSA checked every 3 months, and October will be my first check since the vacation started.

Frequency of PSA Tests?
I’m 8 months post baseline T recovery, and 14 months post
IMRT
. My urologist has suggested every 6 months, but that seems too long from what I’ve read here. Can I get tested every 4 months for the next year, or should I insist on continuing to be tested every 3 months?

Just finished first meeting with radiation oncologist. What he proposed as next steps
In 8 weeks perform 38 treatments of
IMRT
radiation (total of 68.4 Gy) targeting the prostate bed and the lymph area right around the prostate region. This is because of the radical prostatectomy findings that basically saw no spread of cancer beyond the capsule and the low Decipher RP score.
