Chemo or no/not yet?
59 years old. Metastatic/high volume. PSA hit 366 before starting Abiraterone and Degarelix. Start Lupron next week. Negative on DNA test. Will see what PSA looks like next week after a month of ADT. My question chemo, no chemo or wait on chemo? Onco and Rad Onco both seemed to have a very soft attitude

What to do when denosumab course ends?
I have been on denosumab injections since July 2019, so the injection that I am to have next month will probably be my last. During this time I have not had a Dexa scan at all. I am under "shared care" between my GP and the hospital, which is in practice a complete joke, as I have to dairy it, order

I have 6
fractures
of the spine the last few months and treating lymphoma also. I've been told by pharmacy that I would have to pay full price $666 per month till my $2,000 deductible is filled and then pharma care does not cover it anyways.

PSA and Eligard and bicalutamide
My PSA keep rising. Today, PSA went up to 1.1 (Today-03/13/24) from 0.37 (January). Doctor told me to take another Eljgard injection next week. This is the 2nd time. Doctor asked me to stop taking bicalutamide and dutasteride, as they appear not to be effective any longer. He said I solely rely

osteopenia or osteoporosis
I’ve just had a repeat DEXA scan after 5 years. My T scores are spine -0.9 (no change), hip -1.2 (previously-1.4). Femeral neck -2.5 (previously -2.2). Will this result in a diagnosis of osteoporosis. My GP is going to phone me to discuss and is suggesting medication. Is anyone able to give me advice

Update omeprazole versus Famotodine
I'm disappointed because of the gallstones and the threat of more
fractures
but I guess if I don't, my esophagus will get worse next. But being on calcichew D3 is helping the bones...I hope.

Six compression fractures in lower spine
I've had six compression
fractures
in the last three months, all treated with kyphoplasties. On Fosamax and calcium citrate plus vitamin D3. Dexa score is -2.9. Should I be on something stronger?

Should the dose of Dutasteride be increased?
The plot above shows the residual (nadir) level of DHT in men as a function of the daily Dutasteride dose (mg/day). The DHT nadir numbers shown in this plot are: 23.8 pg/mL for 0.5 mg daily dose; 10.2 pg/mL for 2.5 mg daily dose; and 7.3 pg/mL for 5 mg daily dose. Ref: Clark (2004) [see Table

New PSMA avid sclerotic lesions in the left inferior pubic ramus, and bilateral symphysis pubis. Additional new small foci in the left acetabulum and supra-acetabular region. Index focus in the left symphysis pubis with a maximum SUV 20.4. RO will radiate but suggests change from Zytiga to Xtandi.

She has pathologic
fractures
of the sternum and one rib. It seems not much can be done other than rest and time to heal. I did request that he consult with an orthopedic oncologist to see if there is anything more we can do as the pain involved is significant.

PSA doubled in last 30 days after 4 months stable since Provenge. What's next?
Update: PSA remained roughly 1.7 to 1.8 for last 4 months after Provenge but yesterday's 30-day PSA test revealed a jump to 3.190 so something is growing quick. I have been off of all ADT since finishing fourth LU-177 injection five months ago and one SBRT treatment 4 months ago. After LU-177, PSA

chronic pain
I’ve been suffering from longtime, unrelenting chronic pain for more than 30 years. The pain levels have become intolerable. No intervention or painkillers work. I have had major shoulder surgery two months ago which wasn’t a success. I now face having a total shoulder replacement. The pain is excruciating
