Malecare newsletter reports Finish study glucocorticoids (e.g. prednisone, prednisolone, dexamethasone) can lead to castrate resistance

Cabazitaxel plus carboplatin
I take
dexamethasone
, which was prescribed in the summer of 2021 to try to improve appetite. I've also had chronic lower back pain, which apparently is unrelated to PC. I have had scoliosis for some years. I am on Zoladex injections every three months.

Therapies have included amoxicillin/klav + prednisone and now a series of intratympanic injections (I believe
dexamethasone
). My point being, this has been high stress. My world has basically gone off the rails.

Question about Dexamethasone with O&V
I started O infusions, alongisde
Dexamethasone
as a pre-med a relatively high dose of 20 mg (for context, I'm a very small woman!). Following each infusion my neutrophils have dipped again to 0.6 with need for G-CSF to get up to normal levels.

Next treatment options available for advancing disease
I have switched prednisone for
dexamethasone
and haven't had an updated blood test yet to see if has improved. Had consult with oncologist and urologist and I have 3 treatment option available. 1. Docetaxel or Cabazitaxel. 10 treatments.

Reaching the end of treatment, suggestions beyond Docetaxel and Carboplatin.
I read once that the steroid to use with chemo could be
Dexamethasone
1 mg rather than prednisone 5 mg. Is there a clinical reason why? What next?

Fighting a virus
I remember that
dexamethasone
(also a corticosteroid) had some beneficial effects for patients with severe covid. Our experience is only 'anecdotal', but have others had comparable experiences?

Slow rising PSA. Wait and watch?
Should we wait and watch to see if PSA stablises or goes down again, giving Zytiga more time, or should I consult a different doctor and explore changing prednisolone to
dexamethasone
or alternatively Xtandi.

Rituximab- my experience.
I had Rituximab +
dexamethasone
+cyclophosphamide after that for 12 cycles over 10 months. (I had bad nausea with the cyclophosphamide for up to a week after each infusion). Remission again only lasted around 3 months.

Two years and counting
One other trick he mentioned was switching Prednisone to
Dexamethasone
. Two treatment strategies he said were dead, Thorium Th 227 anti-PSMA monoclonal anti body and Amgen AMG 509. Here's hoping for another 2 years.

Mandibular (trigeminal) nerve interference... relief?
In the meantime, this discomfort has not fully responded to any of the meds I am currently taking, and getting great responses from, for all the other pains (back, nerve, etc.) that have arisen recently: gabapentin, oxycodone, tylenol,
dexamethasone
, and THC.

Looking for more wholistic answers to dealing with ITP
Last week they put me on a short dose of
dexamethasone
which helped and now I am getting Nplate injections weekly for a month. From everything I have read, I cannot find a rhyme or a reason or any sure way of treating or predicting this disease.

Started Triplet Therapy this week, new doc, new (better) treatment plan!
We reviewed side effects, and he is agreeable , I prescribed
dexamethasone
and explained the schedule for this. I would like to use the triplet regimen from ARASENS, so I have prescribed darolutamide.

Prednislone and Anxiety
I've been reducing from 20mg pred since November 2022, currently on 11mg. Since Christmas I have had episodes of extreme anxiety, depression, mood swings and tearfulness. My Rheumy this morning suggested swapping to Betamethasone which does not cross the blood/brain barrier. However, when I researched

Doxazosin and what’s next
I have had 2 strokes, one while undergoing chemotherapy, (ovarian cancer, hysterectomy and chemo due to cyst burst) which had to be stopped after 2 sessions due to reactions from Pachtaxel, and
Dexamethasone
(steroids). Was recovering well when woke up with limited movement on right.
