Nearly 7 years after diagnosis - it's decision time.
Mainly for that reason, I've definitely ruled out surgery and have been weighing up the pros & cons of HIFU versus HDR
brachytherapy
as a monotherapy. I would certainly consider SBRT/ proton and other focal therapies too but they are not easily available here in the UK on the NHS.

What Do You Make of This?
Is this saying that targeted treatment with no or limited ADT would have been sufficient, especially if HDR

High risk treatment - go conventional or experimental?
My sense is that despite a one-week sojourn in Seattle it would be more streamlined and perhaps as effective as the androgen deprivation therapy, external beam radiotherapy to the pelvis and an LDR

boost I would get in BC (within or not within a trial).

My 12 month Journey on Active Surveillance
He then went on telling me the details and started explaining my options, surgery, radiation,

Salvage Options after Relapse
I had 1 treatment of HDR
Brachytherapy
at UCSF. PSA consistently dropped (to 0.5) until 2020 when Covid halted my trips back to UCSF. 2021 had me preoccupied with DVT/PE. I’m now reading 3.2. MRI showing still contained in prostate. PSMA Scan next week to confirm.

Dr Scholz' Opinion on Brachytherapy
I haven't received this treatment myself but it could be a future option for me. Dr Scholz says that it is rarely recommended because so few practitioners include it in their bag of tricks. There are politics and economics involved -- other treatments (gasp) are more profitable for urologists! Dr Scholz

Newly diagnosed Stage IV Metastatic Prostate Cancer
I had
Brachytherapy
followed by 25 session of Proton Beam Therapy of the Pelvic area. I thought I was cured. Earlier this year my PSA started rising to 3.9. Gleason Score now at 4+3.

Any experience with these after HDR

? I had HDR 4 months ago. Stage 2, G=3+4, etc. With a less than 10% probably of spread. Still not -0- and I want to make sure. I've limited access to quality healthcare. PSA has gone down to 2 from 9.

Treatment: Unfavorable Intermediate Risk PCA
McBride ruled out
brachytherapy
due to the size of my gland. He thinks I am good candidate for SBRT (MSK Precise) because the disease is confined to the prostate. He also suggests 4 months of ADT because it significantly improves the odds of biochemical recurrence free survival.

Ruminating on Extending ADT&Zytiga beyond 18 months
I am 15 months post HDR
Brachytherapy
with EBRT. PSA Continues to slowly decrease and currently is 0.06 last week. Last scans did not detect bone Mets or soft tissue Mets and suspected node no longer is visible. MO said we can consider stopping ADT at 18 months.
