Interferon Alpha 2-a (PEG) vs 2-b,(Bes) exactly what's the diff?
In the
Besremi
image here, the green part is the IFN, and this could be either 2a or 2b.
Besremi
chose 2b (with Arginine) , Pegasys chose 2a (with Lysine). Does this matter, who knows. It sure does in Jak2 where switching V and F at 617 (Jak2-v617f) makes a life changing diff for us.

interferon resistant
I was on pegasys(max dose) for 3 years with hydrea and now on
besremi
since April with titration of hydrea. Since off hydrea my numbers have been creeping up. I think I'm one of those that are semi-resistant to interferons. Can you continue to use hydrea with
besremi
?

Hello, Recently diagnosed and Specialist wants to follow for a few years till I hit 60 and then begin

Why do we use IFN-α vs IFN-β? Might IFN-β be preferred?
IFN-α is PEG or
Besremi
. IFN-β is sold for MS as “Plegridy”.

What and Why is IFN Pegylation?
Besremi
is different from Pegasys at this circled area. Bes has this single attachment point, PEG has several and they vary. This difference is what
Besremi
claims as their improvement, the attachment is always the same and gives more consistent effects.

increasing HCT despite Besremi
For those of you on
Besremi
, how long did it take to lower your Hct? I have been on it for 2.5 months and my Hct has gone from 41 to 45. I clotted off my liver last summer (Budd-Chairi Syndrome) so my rising Hct is concerning. I am also on Coumadin.

Conversion chart from Peg to Besremi?
So, my option now is
Besremi
. I had significant GI upset on Peg as well so I am nervous, but it is my only option at the moment. All of your tales of doing well on
Besremi
have me hopeful. I remember that someone had posted a conversion chart on this site. Perhaps from Mesa?

HISTORY: Diagnosed with PV 4 years ago; JAK2 positive with very low erythropoeitin; I am now 64 years old; Only phlebotomy and aspirin for 4 years until
Besremi
in Feb 2022. Most concerning before
Besremi
was platelets of 1100. Allele Burden was 70% before

Allele Burden Question
I went back and reviewed my bone marrow biopsy results taken to establish as a baseline prior to
Besremi
for reference regarding allele burden. There were two mutations present. JAK I already knew about. The other listed is ASCL1. Two questions.

I am about to start
Besremi
. Just looking for some reassurance. I am 40 year old working mom. I was diagnosed over a year ago. My HCT has been great, 8 mos in between my last phlebotomy, but my PLTs are crazy high and my wbcs are coming up. Just hoping someone can tell me what to expect.

I am about start
Besremi
. I just wanted to know how people are doing on it. I am hoping to get it this week. My Hct has done well from the start, took 8mos before I had to have another phlebotomy, but my wbcs and platelets are really high. I just want someone to reassure me!

Is 69.7 % my allele burden?
Testing in January of 2022 just prior to the first
Besremi
injection. Is Mutation Frequency the same as Allele Burden? Please see attached lab results Thanks.

PEGASYS and Besremi Site injection pain
I want to stay on
Besremi
because of the remission capabilities that no other drug offers. My question is has anyone else gone through this and am I having adverse reactions to Interferon itself? Thank you kindly. Sam

PV with MS
I am still waiting to get
Besremi
. Insurance is still not wanting to cover it. I had a MRI and found out I have MS too. I know both can be treated with intferon. Anyone out there by any chance have both and have an idea if they can be treated with the same drug. Thanks

Update 7.0
The docs did not feel that an increase in
Besremi
dose nor phlebotomy was needed at this point. We will continue to monitor There was another liver anomaly on the last CMP (below). ALT = 124. AST = 87.0. Alk Phos = 167.

starting on Interferon
I’m wondering has any one in uk gone straight onto
Besremi
? I’ve read here that it’s proving to halt progression of PV and reduce symptoms and is more tolerable ie has less symptoms itself vs Peg. My symptom symptoms are worsening especially fatigue.
