Besremi

I have been on besremi for almost 2 years. I have PV and ET. My AB at its highest was 58 it is now 25. I am wondering if others are able to reduce besremi or if anyone has reduced their AB to zero. Does that mean you no longer have PV?

due to the global shortage of Pegasus I have been moved temporarily onto besremi . The side effects of fatigue have hit me like a train . I had no fatigue in Pegasus . Has any one else experienced this and if they have was it temporary .

Hi I have been on besremi for 18 months and my hematocrit is finally becoming controlled. Has anyone gone down to monthly shots? Has anyone been able to track their mutation frequency?

Since started Besremi in early 2023 my JAK2 has gone from 51.89% to 13.17% I hadn’t realized that drops in hematocrit and WBC could potentially indicate progression to myelofibrosis. I’m hopeful that these numbers will start to climb after reducing my Besremi dose.

I'm interested in others who may have had something similar with auto-immune issues on Besremi. Apologies for the saga.

Many of us are likely to be moving over to Besremi within the next month or so. Are there many here who have already done this and can they please share their experiences re equivalent dosages and any side effects. Also how long a gap between stopping Pegasys and starting Besremi?

My question is, has anyone gone through BESREMI to the 500mcg and still had symptoms? Did you switch to JAKAFI? Did it help?

He claimed that the efficacy of older IFNs is only about half of Besremi, based on what the manufacturer Pharmaessential claimed in this slide, which seems to that less than 50% of patients using other IFNs achieve CHR, but Besremi is close to 80%.

I would assume that to get Besremi, the doctor would have complete some paperwork with the insurance company beyond just handing me a prescription. I would be interested if anybody could give more specific information. Thanks.

Wondering if you have ever tried Besremi or if there is any research that addresses that. With ASXL1, if I do progress, it is an aggressive gene with poor clinical outcomes.

My preference is rather Besremi, but my hematologist is totally unfamiliar with interferon…

Also wondering if Besremi would be better suited for my sluggish thyroid. Does anyone know if Besremi is a little gentler on the thyroid? Any input on your experiences would be greatly appreciated.

I just wanted to say Happy Wednesday and Besremi update. I am now down to 175mcg every other week. WBCs came up to 3.0, hct 36.5, PLts 183! Neutrophils up to 2.1, lymphocytes 0.7. Feeling great! Thyroid is under better control and all other labs looked pretty good!

I am now 9 months into the Besremi trial. I started on February 20th, 2024, with the first dose of Besremi of 100mcg. I increased 50 mcg every two weeks until reaching 500mcg on June 11th.

So far, the allele burden continues to drop: From 56% in April 2022, when I started , to 7.96% in January 2025. For some reason 2024 was left off this report: 24.67% in Feb 2024 and 13.17% in June 2024. No disease burden at this point. Maybe some fatigue but less than before.

Just been to pick up my first couple of syringes of Besremi but Pharmacy couldn’t determine the equivalent dosage level. I showed them Hunter’s screenshot of Dr Mesa’s conversion table but they wanted something from the FDA. Hence asked me to come back tomorrow whilst they work on it.

Today I saw news on PharmaEssentia's website stating that they have applied in the United States to launch a pen Injector for Besremi. I think this is a good development?
