Switch from Peg to Besremi

Finally got my first script for Besremi. It took a while and a lot of hoops for my MPN specialist to jump through to get temp approval to move from Peg to Besremi given the global shortage of Peg. After a couple of letters and phone calls and a meeting with the exceptions committee, I was finally notified last week that I had been approved to receive Besremi. Interestingly enough they gave approval until July 2027..their thinking that we'll see how fast the company can restart Peg production. I got to experience how lucky we are when we have a specialist who is also an advocate..the committee kept pushing toward anagrelide (they had my records that I was HU intolerant). My MPN specialist kept arguing that this went beyond just thrombosis risk - that I had responded very well to IFN therapy (all bloods within normal limits) and without side effects and that given my age we were looking at potentially disease modifying therapies....and while Besremi was not approved for ET (or pre-fibrotic PMF) as it was for PV..that Peg is also off-label use for ET and they are essentially the same medicine (we all know of course that there is a significant cost differential)....

Anyway at the end of the day he was successful in arguing the case and I now have Besremi. I took my last shot of Peg on Saturday and start Besremi in two weeks (at the converted dose). Will be back on bi-weekly blood test for the first 4 shots (2 months) as my doctor wants to closely monitor the change over (he doesn't expect to see anything but I agree I'd rather be more cautious than not)....hopefully the Bes will work for me as it has for others here and as well as the Peg has....

Good for you for also advocating for yourself. You are fortunate to have a hematologist who understands why Besremi is superior to anagrelide for your care and is putting your best interests first.

If your experience is like mine, you will find no difference between Pegasys and Besremi in terms of efficacy or tolerance.

Hope you doing good on the Besremi Solyesh. Just read your post, would be brilliant if all Consultants were like yours Amazing :) I have got 1 interferon injection left for this week, waiting on call re: next prescription mmm Have A Brilliant day You and all MPN buddies :)
