Hi! I have been on besremi for almost 2 years. I have PV and ET. My AB at its highest was 58 it is now 25. I am wondering if others are able to reduce besremi or if anyone has reduced their AB to zero. Does that mean you no longer have PV?

My JAK2 VAF has reduced from 38% to 10% on a 175mcg dose of Besremi. I hope it will go lower but cannot raise my Besremi dose to achieve that outcome.
It is reported that about 25% of people on Besremi achieve a complete molecular remission (CMR). There are a number of factors that influence this outcome, including dose and non-driver mutations.
It is not yet clear what it means to achieve a significant reduction in VAF or a CMR. There is not evidence that a CMR is a cure for PV. This is an area of active research. At this point I think the best way we can put it is that a reduction in VAF is a good thing but we cannot prove how good it is. Yet. Hopefully we will know more in the near future.

I’m on Pegasys, not Besremi, and have found out this week that my AB has reduced from 88% when I began Peg, to 22% after 4 years of interferon treatment.
I asked my consultant if my consistently ’normal’ blood counts meant I was in molecular remission, but apparently she’d need to see an AB of 0% for that.
Instead, my results are referred to as a’a good molecular response’.
