Emotional instability after stopping lupron and abiraterone
After an initial few months of positive improvements in every sector, my guy started into a mental health/cognitive decline to the point where he is now dependent on gabapentin he takes for other reasons in order to maintain any emotional stability. He says his short term memory is also getting worse

My PSA is now at 47 after a high of 160 and a low of 5 after starting ADT with Firmagon, Abiraterone and Prednisone about 9 months ago
I was put on Firmagon in January and my testosterone responded almost immediately to less than 7, but the PSA kept rising, so I was put on
Abiraterone
and Prednisone in March. The PSA went from 160 down to 5 within about 3 months.

Pleural effusion post second round(18 cycles) of Taxotere
PSA was rising on
Abiraterone
too after having failed Enzalutamide one and half year ago. Full history in profile. He was diagnosed with mild pleural effusion in lungs in PSMA PET scan after chemo. Fluid(about 700ml) was drained and cell blocks and tests did not find any cancer cells in fluid.

Distant Lymph node treatment options?
Started ADT with Eligard shot,
Abiraterone
, Prednisone,Pantoprazole on Oct 15th. On 23rd of November had new tests and PSA dropped to 6.8. I was elated by such a drop and upon a visit to my Dr he suggested radiation therapy.

ADT stat, then the first of 15 Taxotere chemos got PSA to 0.7 Started Metformin/Lipitor in 2016 with a PSA nadir of 0.1 in the Summer of 2017 during an 18 month hormone "holiday"

How common is Nocturia with PCa?
Is this caused by PCa,

From the janssenlabels product insert: " Metastatic castration-resistant prostate cancer: • ZYTIGA 1,000 mg orally once daily with prednisone 5 mg orally twice daily. (2.1) Metastatic castration-sensitive prostate cancer: • ZYTIGA 1,000 mg orally once daily with prednisone 5 mg orally once daily.

Currently Post HDR Brachytherapy, EBRT with ongoing ADT+

. I am stable and PSA continues to slowly go lower current is 0.13 two months post EBRT and 4 months post HDR Brachy.

Rising PSA after chemo, next steps?
Started degarelix, had 6 sessions of docetaxel, added
abiraterone
in August, consolidative SBRT in September. Lowest PSA was 0.99 right after chemo, in August. In September, 1.12, October 1.87, and now in November it is 4.16. His Testosterone levels remain <7. (Switched to Orgovyx 6 weeks ago.)

I assume the choices are immediate systemic treatment (ADT, perhaps in combo with

again...or something else?) or waiting until the recurrence is detectable on the recently approved PSMA scans.

PSA is increasing with Docetaxel, what should we do now ?
he stoped Casodex 50mg (bicalutamide) and switched to Zytiga 500mg (
Abiraterone
acetate), 1000mg daily along with Solupred 5mg (Prednisolone) only 5mg daily.

Zytiga and Prednisone dosage
I’m currently taking 5mg prednisone daily along with my 1000mg
abiraterone
. I’ve noticed that my blood pressure has been creeping up… averaging maybe 140/80. Is this a case where I would maybe be a candidate for 10 mg of prednisone instead of just 5mg?
