Lynparza after Enhertu?

Question about PET scan results. This is my 2nd PET on Enhertu for metastatic breast cancer with great results after the 1st scan. I just received my results from this morning and all organs and liver look clean but there has been some low SUV level skeletal progression. I have a meeting with my oncologist tomorrow but she said we may try Lynparza and stop Enhertu due to the several new areas. I’m ER+, HER2 low, BRAC2+.

Has anyone here started lynparza after Enhertu or Taxol with good results?

Verzenio - did nothing

Taxol- worked for 3 months

Enhertu - worked for 5 months

There are multiple new foci of hypermetabolism within the skeleton. For example, maximum SUV of the T3 vertebra is 3.6. Maximum SUV of the sternum is 2.9. Maximum SUV within the L4 vertebra is 3.6. Maximum SUV within the right iliac wing is 3.0. Maximum SUV within the sacrum is 3.1.

Are you taking anything like Xgeva? I wonder if trying that or some other bone met direct treatment might be better than switching meds. Just my opinion...I typically argue with a med change for as long as possible. My bone mets are fairly stable - I think - due to Xgeva (which dont get me wrong has its own problems) and a three dose "hit" of radiation to a bad spot on my hip. Hope this is helpful.

I totally agree… Reported SUV isn’t that high and visceral mets are usually more difficult to treat than bone mets. Enhertu is working with liver, so I’ll try to ask if they can consider Xgeva and/or radiation.

one thing… did your onc talk about trying Talazoparib? It’s a PARP inhibitor like Olaparib(Lynparza), but it’s new and gives good results as far as I’ve read.

I had Talazoparib for 4 months last year. It had good control and no obvious side effects. Everything looked good and shrinking until I had pericardio effusion. After testing, my onc said I have multiple type. I was HR+Her2-BRCA2+ before using Talazoparib. Then, they said I also has HR+Her2+ so had progression. Then I started a trial with Enhertu plus experimental drugs. It worked for 9 months. I am not sure does that mean parp inhibitor doesn’t work on Her2 part? Cancer is very complicated and everyone is different reacting to the drug. But we all agree if it’s no major progression, we want to stay in one drug as long as possible. Keep us updated and good luck to your new treatment.

I was tested for BRCA last year and my onc talked to me about this treatment. But I tested negative… If you don’t mind keep us updated.

Hope your appt went well. I was also recently put on Lynparza because Ibrance stopped working for me. Er+, Her- , pr-, BRCA2+. Increased lesions in liver and sternum with last CT in May. Also on Xgeva.

The big difference in switching as far as s/e is indigestion and fatigue. Also dry skin. I do take with food and am making sure I drink enough water (hence my name: Aquamoron). I have only been on this 6 weeks and am hoping my body adjusts.

From what I read, Lynparza is the newer gold standard for BRCA patients and has good results. Fingers crossed I will respond well to this. Yes, it’s a pain taking 4 tablets a day but in the upside even though I’m tired I’m not experiencing the brain fog I had when I was on Ibrance.

UPDATE: after meeting with my oncologist we are going to continue with 2-3 more Enhertu treatments and then rescan using the same PET machine as well as adding in an AI. The machine I used was a brand new machine which might be more sensitive than other machines I have used in the past. We will watch markers and labs and if there is anything alarming we will scan right away. I truly appreciate all of your comments.

I think this is the wise choice to stay on the same treatment! My SUV has been double those amounts for a long time and I'm fine (with exercise to keep me strong). It is always better to squeeze as much time as possible out of these drugs, so you don't run through all your options too fast. I had bone mets mostly, and a tiny liver spot when dx with mets. First I took iBrance/faslodex/Xgeva for nearly 3 years but there was bone progression over the last few months. Then I switched to Lynparza/Xgeva, which interestingly did not work so well for me. I only took it a year and then got liver mets anew and multiple! I've been on a trial since December but it's not working either (a combo of these 3 drugs) so I'm just about to come off and start something new. Meeting doc tomorrow!

Hi. I’m writing because I too have had to try many different treatments in a short period of time. I wish a drug would work for more than a year! I’m actually about to get my first infusion of Enhertu next week. Nurse visit today. This is not an easy journey so I wish you and the community any good days. Please let us know how it’s going. I too am just beginning Enhertu after many other treatments.My onc tells me if Enhertu fails my next choice would be clinical trials I would appreciate anything you can tell me about your Enhertu experience.

Sorry you still have progression😞I will begin Enhertu infusion tomorrow.My oncologist seemed to say if Enhertu failed all I had left was finding a trial?
