I'm 2 months post robotic-prostatectomy. All went very well regarding surgery as is, my continued recovery. The one thing which I can't understand or work out is masturbation incontinence. 90% now I'm dry, just wearing a small dry pad. Doing my Kegel exercises every day, 3 times a day.

On trying a little gentle masturbation, with only semi erect penis, as soon as I start, I'm constantly leaking a lot of urine, despite emptying my bladder well before I start.. I can virtually soak through a small towel beneath me during the time Im attempting to masturbate. On the masturbation front, I'm still really struggling to get even a semi erection yet, but it's early days I suppose. I'm taking my 5mg of cialis every day as prescribed by my consultant. Ive got my first PSA next week following the surgery.

Any hints or tips guys please on the above problem.

It's called "climacturia" and it's fairly common (about 20%) after RP. A few tips I've picked up from support groups:

• Restricting fluids for a few hours before sex, especially coffee, tea, Coke, alcohol and fruit juices

• peeing before sex (sit down and rock around or in the shower - whatever it takes to fully empty the bladder)

• a constriction band around the base of the penis (also helps maintain erection)

• lay down on your back and don't change position

• use a condom

• get rubber sheets, and just enjoy water sports

Unfortunately, I suspect that ED meds (like Cialis) may make it worse.

it’s a problem I dealt with a lot. Even worse when I had an orgasm. I ejaculated urine. Now that I’m doing the ADT I can’t even get aroused so the urine leaking during masturbation is no long a problem . I’m sorry brother I feel you

Thank you so much for your reply. I'm sorry to hear of your continuing fight against this disease. I totally agree, its quality of life that has to count, not filling us full of horrible drugs with foul side effects.

8 years out. Get pretty hard but not enough to penetrate. Leaking with sex has been a consistent problem. Draining bladder and limiting fluids helps a lot. I also found a playmate who enjoys it. Adds some nice spice.

Thank you for the feedback, Wish I could find a mate who enjoys my constant masturbation urine leaking. I find it distressing and annoying whilst constantly being constantly wet will trying to get anything resembling an erection and then trying to masterbate. Hoping it improves with time and exercises.

For me it has gotten much better. I also decided to embrace it. Always have a towel handy. With an orgasm, it does feel like ejaculation which I very much miss. I have embraced and learned to catch that shooting piss…and on occasion plan for it. I also have my playmate who enjoys it and I feed him with abundance.

I'm no way near orgasm yet after just a few weeks since my robotic-prostatectomy, but I'll keep persevering!. For now, I'll remain happy getting semi hard, and leak loads of urine.

Hello, I do have a medical pump thanks. That's working all fine, it's just as soon as I start trying to masturbate I'm just leaking urine constantly despite emptying my bladder and reducing fluid intake beforehand. I can really wet a small towel on each attempt?. Hey Ho!!

Hi Neil

This is part of post prostatectomy that the NHS surgeons never tell you about. It does improve as your body heals and Kegels over time will help that. I found that it eased a lot after about a year but I would still suddenly let go during foreplay even though my incontinence otherwise was under control. Unless I had been consuming alcohol, when it would be difficult to get under any sort of control. Brings a whole new meaning to a piss-up eh?

I have accepted it now and just prepare for the occurrence with towels and waterproof mats so as not to soak the bed clothes. As time progresses Neil you will adapt and things like cock rings do help.

I have just made it part of my sexual life now and to be honest I find shooting a stream of urine more satisfying than a dry orgasm. Still miss the feeling of something coming out of the end of my cock. However it does help if you have a partner that does not freak at the sight of urine instead of cum.

Take it slow and just let your body heal and experiment with some of the solutions mentioned here.

Thanks Brian, this information is so helpful. For now, and only a few weeks post robotic-prostatectomy, I'll persevere with a gentle wank, lots of leaking pee. I'm doing Kegel exercises 3 times a day. Pee control on a normal level is getting very good. Just when I try to play with "him", downstairs I'm leaking!!!.

Remember it can take up to two years for your erectile function to return, so if you are getting even a partial erection at just two months you are way ahead of the curve. My understanding is that on average most of us don't start to see any return of function until the 1 year mark. That has been my experience. I am at 1 year and with a snug cockring I can actually get an almost close to normal erection as I get close to orgasm. I am doing all the rehab in addition to intracavernosal injections in the interim, but it was a thrill to see some wood return. I have also started to wake up to erections at night, another sign of some return. Bottom line, just hang in there, mate. It is early.

One option for dealing with urine leakage is a cockring made by Firmtech. One part of it constricts the base of the penis, so not only does it help with erection maintenance it constricts the urethra to help avoid urine leak.

Thank you. Tom. Appreciate the guidance. I'm full aware 7 weeks is not long, so will be patience and let the healing process continue. My partial erection is obviously still only around 20%, but at least it's obvious a few nerves down in that ex, prostate area, must be starting to recover a little. I think it will be as you say, probably at least a year, before I get any kind of decent enough erection, allowing me to dry orgasm. Thanks again.

how long has it been since your surgery? I was the same, leaking and couldn’t get very hard. My urologist said I was not giving myself enough time to heal… I did much better after 6 months.. I now don’t leak as much, only a few drops if I’m tired, and I get relitively hard..

I'm not expecting too much yet to be honest. I'm trying not to be impatient.

Just experience with trying to get hard without the vacuum pump the hospital gave me. I'm not keen on it at all. Feels so unnatural.

But I do realise it supposed to help prevent penis shrinkage. I really must start giving it a go to retain my average size length.

On trying to masturbate now, with a fairly flaccid penis, I'm just constantly leaking urine. It gets everywhere, runs down my testicles, on my hands , and towel I put between my legs.. So after a while I just lose interest and clean up.. Neil

It's been a while since I've posted. I had RP by DaVinci in 2008. Never got to 0 and through an insurance shafu and a gap in testing it went back up to 13 by 2014. We did SBRT Cyberknife to a seminal Vessicle that seemed to be the extent of it. They put me on Leuprolide which dropped my libido and made me a little tired. PSA went up again and they did a repeat. in the spring of 2022 I began experiencing visual and perception deficits on my left side. Walking into things and people--sideswiped two cars.

The opthalmologist sent me for an MRI and CT scan. Part way through the tech came over and took my hand and said they'd had a preliminary look and he was to walk me down to the ER. 17 hrs later I had an Appt' with a neurosurgeon and 19 days after that they opened the back of my skull and took out a 6 x 4 x 2 cm cystic tumor that was pressing on the occipital lobe and causing all the weirdness. They showed me a video of the thing spilling yellow fluid all over. They determined that it was PC that had metastasized to the cranium and took it out. At this point they added Xtandi (enzalutamide) as it's the only one that crosses the blood/brain barrier. I've lost all my body hair, I get tired easily and grew a belly since the surgery. Libido has totally gone.

I'm into the 2nd yr now and the scans and PSA are all good. It sounds pretty grim, but I feel blessed in that PC doesn't go to the brain until it has pretty much spread through the rest of the body. Not so with me. Body scans were and remain clear. ("1 in a million" per the hot Venezuelan oncologist. I can't get it up and have to make an appointment to remind me to have a wank, but doesn't change how I react to good looking men. ) I'm 81 and haven't had a regular sex partner for some time.

That's the long story. I have not used pads since a few months after surgery. I was able to get a decent erection masturbating and got in 4-6 wanks a week until now. I irregularly shot varying amounts of pee. The most disconcerting thing was leaking during arousal or just the thought of sex. I'm stubborn about not using pads since leaking is usually light and irregular, but on occasion I have leaked enough to show. When I make a point to of masturbating I get enough of an erection to hold onto and do shoot pee.

I've had several sweet liaisons up until this last thing when I just don't think of it, to tell the truth. Thanks, bill

I am still doing my Kegel exercises and ny incontinence is slowly improving.

When attempting masturbation, I always ensure that I try and fully empty my bladder first. It doesn't seem to make any difference at all.

I normally masturbate in the kneeling position, positioning myself over a large towel. I take it steady, and calm. I'm still not getting an erection at all despite taking the prescribed 5mg of Cialis every day now for 3 months. So have to do the act with a very flacid penis. The towel always ends up wet through.

I try to casually masturbate for up to 1 hour, but literally start squirting spurts of pee almost constantly throughout the session. I have, now managed a dry orgasm twice, since my surgery on 5th February.

I'll keep my spirit up!.
